摘要
目的探讨糖尿病患者趋化因子-20(C-C chemokine ligand-20,CCL-20)与急性脑梗死的关系,为急性脑梗死的发病机制和防治措施提供依据。方法选取2017年4月1日-2019年10月31日期间某三级甲等综合医院急诊科就诊的糖尿病合并急性脑梗死患者92例和入住内分泌科的糖尿病未并发急性脑梗死的患者82例作为研究对象。完善体重指数、血压、血脂、血糖等检查,对2组血清CCL-20水平比较,血清CCL-20与收缩压、高密度脂蛋白胆固醇进行相关性分析。结果糖尿病合并急性脑梗死组血清CCL-20为104.34±18.78pg/mL,糖尿病组为40.88±12.05pg/mL。糖尿病合并急性脑梗死组的血清CCL-20水平较糖尿病组明显升高,差异有统计学意义,t=6.193,P<0.001。在调整了相关混杂因素后,糖尿病患者血清CCL-20与收缩压正相关,β=0.995,P<0.001,与高密度脂蛋白胆固醇负相关,β=-18.524,P=0.004;并且血清CCL-20升高是糖尿病患者发生急性脑梗死的独立危险因素,OR=0.555,P=0.043。结论血清CCL-20水平升高是糖尿病患者发生急性脑梗死的独立危险因素。
Objectives To investigate the relationship between C-C chemokine ligand(CCL)-20 and acute cerebral infarction in diabetic patients,and provide the basis for the pathogenesis as well as prevention of acute cerebral infarction.Methods A total of 92 diabetic patients with acute cerebral infarction and 82 diabetic patients without acute cerebral infarction were recruited from clinics.Serum CCL-20 level was measured by ELISA.Body mass index(Body mass index,BMI),blood pressure,blood lipids,and blood glucose were measured.Results Serum CCL-20 in diabetic patients with and without acute cerebral infarction group was 104.34±18.78 pg/mL and 40.88±12.05 pg/mL respectively.Significant difference was found between the two groups(t=6.193,P<0.001).Moreover,serum CCL-20 was positive correlated with the systolic pressure(β=0.995,P<0.001)and negative correlated with the high-density lipoprotein cholesterol(β=-18.524,P=0.004)in diabetic patients.The result of binary Logistic regression showed that high CCL-20 level was the independent risk factor of acute cerebral infarction in diabetic patients(odds ratio=0.555,P<0.001).Conclusions A high CCL-20 level was the independent risk factor of acute cerebral infarction in diabetic patients.
