摘要
目的分析膀胱癌患者外周血循环肿瘤细胞(CTC)中生存蛋白(survivin)的表达及意义。方法选择空军军医大学唐都医院于2019年1月至2020年11月期间收治的54例膀胱癌患者为研究对象,采集其外周静脉血,采用CanPatrol技术检测CTC,免疫组化法检测survivin水平,比较不同肿瘤直径、病理分级、肿瘤分期(T分期)、肿瘤数目患者的外周血CTC、survivin的表达差异,以及不同类型CTC中survivin阳性率的差异。结果54例膀胱癌患者中25例检出CTC,阳性率为46.30%,主要包括13例上皮型CTC、7例间质型CTC、5例为混合型。上皮型CTC中,7例为survivin阳性表达,占53.85%;间质型CTC中,均为survivin阳性表达,占100.00%;混合型CTC中,4例survivin阳性表达,占80.00%。间质型survivin阳性率显著高于上皮型和混合型CTC,差异有统计学意义(P<0.05)。肿瘤直径<3 cm、分期<T2、单发肿瘤的CTC检验阳性率更高,差异有统计学意义(P<0.05);不同病理分级之间的CTC检测阳性率比较,差异无统计学意义(P>0.05)。肿瘤病理分级中高级别、分期≥T2的survivin检测阳性率分别为24.07%、25.93%,明显高于低级别、分期<T2的9.26%、7.41%,差异均有统计学意义(P<0.05);不同肿瘤直径、肿瘤数目的survivin检测阳性率之间比较差异均无统计学意义(P>0.05)。结论膀胱癌患者中,间质型CTC的survivin阳性表达率较其他型别明显更高,且其表达与膀胱癌患者病理分期、分级之间具有一定相关性。
Objective To analyze the expression and significance of survivin in peripheral blood circulating tumor(CTC)cells of patients with bladder cancer.Methods A total of 54 patients with bladder cancer,who admitted to Tangdu Hospital from January 2019 to November 2020,were selected as the research subjects,and peripheral venous blood was collected.CTC was detected by CanPatrol technique.The level of survivin was detected by immunohistochemistry.The difference in tumor diameter,pathological grade,tumor stage(T stage),tumor number,blood CTC and the expression of survivin were compared,as well as the difference of survivin positive rate in different types of CTC.Results Among 54 cases of bladder cancer,25 cases were positive for CTC with the positive rate of 46.30%(25/54),including 13 cases of epithelial CTC,7 cases of interstitial CTC and 5 cases of mixed type.In epithelial CTC,7 cases were survivin positive,accounting for 53.85%;in interstitial CTC,all were survivin positive,accounting for 100.00%;in mixed type of CTC,4 cases were survivin positive,accounting for 80.00%.Survivin positive rate of interstitial type was significantly higher than that of epithelial type and mixed type of CTC(P<0.05).The positive rate of CTC test of tumor diameter<3 cm,tumor stage<T2,and single tumor was significantly higher(P<0.05);there was no significant difference in the positive rate of CTC test among different pathological grades(P>0.05).The positive rate of survivin in patients with high-grade and tumor stage≥T2 were 24.07%and 25.93%,respectively,which were significantly higher than9.26%and 7.41%in in patients with low-grade and tumor stage<T2(P<0.05);there was no significant difference in the positive rate of survivin among patients with different tumor diameter and tumor number(P>0.05).Conclusion The positive expression rate of survivin in interstitial CTC is significantly higher than that in other types of bladder cancer,and there is a certain correlation between the expression of survivin and the pathological stage and grade of
