摘要
目的研究全腹腔镜辅助下改良overlap吻合术对近端胃大部切除(PSG)患者消化道重建的作用效果及安全性。方法选择2019年3月—2020年6月在盘锦市中心医院接受PSG手术治疗的胃食管结合部的腺癌患者88例作为观察对象,随机分为观察组和对照组各44例。两组均常规实施全腹腔镜辅助下的近端胃大部切除术式。在消化道重建阶段,观察组实施改良overlap吻合术,对照组实施食管和残胃前壁的常规吻合。随访6个月,对比两组手术相关指标、生活质量和术后并发症情况。结果观察组的手术时间和食管胃的吻合时间,术中出血量和住院时间与对照组相比,差异无统计学意义(P>0.05)。治疗后两组的生活质量均较治疗前升高,差异有统计学意义(P<0.05),但组间差异无统计学意义(P>0.05)。观察组术后并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论全腹腔镜辅助下的改良overlap吻合术对PSG患者消化道重建的作用效果较好,且安全性较高,还可改善其生活质量,值得临床进一步推广。
Objective To study the function effect and safety analysis of total laparoscopic assisted modified overlap anastomosis on gastrointestinal reconstruction in patients with proximal subtotal gastrectomy(PSG).Methods A total of 88 patients with gastroesophageal junction adenocarcinoma treated in our hospital from March 2019 to June 2020 were observed.According to the random number table method,all patients were divided into observation group and control group with 44 cases in each.Both groups routinely performed a total laparoscopic subtotal gastrectomy,At the stage of postoperative digestive tract reconstruction,modified overlap anastomosis was performed in the observation group,the control group performed routine anastomosis of the anterior wall of esophagus and residual stomach.After six months follow-up,the surgical indicators,quality of life,and postoperative adverse reactions were compared between two groups.Results The operative time,anastomosis time of esophagus and stomach,the intraoperative blood loss,and hospitalization time displayed no significant differences between two groups(P>0.05).After treatment,the quality of life of the two groups was significantly improved than that before treatment(P<0.05),but there were no differences between two group(P>0.05).The total incidence of postoperative adverse reactions in the observation group was significantly lower than the control group(P<0.05).Conclusion The effect of total laparoscopic assisted modified overlap anastomosis on gastrointestinal reconstruction in patients with proximal subtotal gastrectomy is better,and has high safety.It also improves their quality of life,It's worth promoting.
