摘要
目的分析促红细胞生成素(EPO)联合氨甲环酸(TXA)对股骨转子间骨折患者围术期失血量的影响。方法选取2019年6月至2020年6月我院收治的186例股骨转子间骨折患者,均行PFNA手术治疗,按照术前使用药物不同分为观察组(n=96)和对照组(n=90)。观察组术前静脉滴注EPO和TXA,对照组术前仅静脉滴注TXA。比较两组的失血量(总失血量、隐性出血量)、输血率、血液指标[血红蛋白(Hb)、红细胞积压(Hct)]。结果观察组的总失血量、隐性出血量及输血率均明显低于对照组(P<0.05)。术前2 d,两组的Hb、Hct比较差异无统计学意义(P>0.05);术后3 d,两组的Hb、Hct均明显低于术前,观察组的Hb、Hct明显高于对照组(P<0.05)。结论术前滴注EPO联合TXA可有效减少股骨转子间骨折患者的围术期失血量,降低输血率,促进患者术后恢复,值得临床推广应用。
Objective To analyze the influence of erythropoietin(EPO)combined with tranexamic acid(TXA)on perioperative blood loss in patients with femoral intertrochanteric fractures.Methods 186 patients with femoral intertrochanteric fractures admitted to our hospital from June 2019 to June 2020 were selected and given PFNA surgery.They were divided into observation group(n=96)and control group(n=90)according to the different preoperative medication.The observation group received intravenous infusion of EPO and TXA before surgery,and the control group only received intravenous infusion of TXA before surgery.The blood loss(total blood loss,recessive blood loss),blood transfusion rate and blood indicators[hemoglobin(Hb),hematocrit(Hct)]were compared the two groups.Results The total blood loss,recessive blood loss and blood transfusion rate of the observation group were significantly lower than those of the control group(P<0.05).2 days before surgery,no statistical difference was found in the Hb and Hct between the two groups(P>0.05).3 days after surgery,the Hb and Hct of the two groups were significantly lower than those before surgery,and the Hb and Hct of the observation group were significantly higher than those of the control group(P<0.05).Conclusions Preoperative infusion of EPO combined with TXA can effectively reduce the perioperative blood loss,lower the blood transfusion rate,and promote the postoperative recovery of patients with femoral intertrochanteric fractures,which is worthy of clinical promotion and application.
