摘要
目的分析自身免疫性肝炎(AIH)患者血清免疫球蛋白(Ig)水平与肝脏炎症程度的关系,为后续AIH的治疗提供指导。方法分析2018-01-01-2019-12-31河南大学淮河医院接受治疗的83例AIH患者的临床资料,根据肝脏活检结果将患者的肝脏炎症程度分为轻度、中度和重度,并统计所有患者的一般资料,包括性别、年龄、发病情况、合并自身免疫性疾病(AID)情况、病程与血清IgG、IgA和IgM水平。结果病理检查结果显示,83例AIH患者中,轻度肝脏炎症26例(31.33%),中度40例(48.19%),重度17例(20.48%);轻度肝脏炎症患者血清IgG水平为(11.38±4.49)g/L,低于中度(17.56±5.21)g/L和重度(23.78±6.3)g/L,差异有统计学意义,F=29.275,P<0.001;中度肝脏炎症患者血清IgG水平低于重度患者,差异有统计学意义,P<0.05。多因素Logistic回归分析结果显示,IgG过表达(OR=1.278,95%CI为1.144~1.429,P<0.001)是AIH患者肝脏炎症加重的影响因素。结论AIH患者血清IgG过表达会导致肝脏炎症程度加重。
Objective To analyze the relationship between serum immunoglobulin(Ig)level and degree of liver inflammation in patients with autoimmune hepatitis(AIH),and to provide guidance for subsequent treatment of AIH.Methods The clinical data of 83 patients with AIH who received treatment in Huaihe Hospital of Henan University were analyzed.According to the liver biopsy results,the degree of liver inflammation of the patients was divided into mild,moderate and severe.The general data of all patients were analyzed.Including gender,age,onset,combined autoimmune disease(AID)status,disease duration and serum IgG,IgA and IgM levels.Results The pathological examination results showed that among the 83 AIH patients,there were 26 cases of mild liver inflammation(31.33%),40 cases of moderate liver inflammation(48.19%),and 17 cases of severe liver inflammation(20.48%).The serum IgG level of mild liver inflammation patients was(11.38±4.49)g/l,lower than that of moderate(17.56±5.21)g/l and severe(23.78±6.3)g/l,the difference was statistically significant(F=29.275,P<0.001).The serum IgG level of patients with moderate liver inflammation was lower than that of patients with severe liver inflammation,and the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that IgG overexpression(OR=1.278,95%CI:1.144-1.429,P<0.001)was an influential factor for liver inflammation in AIH patients.Conclusion The overexpression of serum IgG in AIH patients can lead to aggravation of liver inflammation.
