摘要
目的基于Meta分析系统评价右归丸治疗原发性骨质疏松症的临床疗效。方法检索中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据库)、中文科技期刊数据库(维普网)、中国生物医学文献数据库(CBM)、PubMed、Embase、Cochrance Library建库至2021年1月7日右归丸治疗骨质疏松症的随机对照试验文献,根据Cochrane风险评估工具对纳入文献进行质量评价,提取数据并采用RevMan 5.3软件进行Meta分析。结果共检索到260篇文献,最终纳入文献9篇。总样本量为778例。Meta分析结果显示:右归丸组总有效率高于西药对照组(RR=1.25,95%CI:1.11~1.40,P=0.000 1),亚组分析结果显示右归丸联合西药组总有效率(RR=1.34 95%CI:1.21~1.48)高于单纯右归丸组(RR=1.13,95%CI:0.90~1.43);右归丸治疗组骨密度显著高于常规西药治疗组(MD=-1.99,95%CI:-2.33~-1.65,P=0.000 01)。右归丸治疗组骨素钙低于西药组(MD=-1.99,95%CI:-2.33~-1.65,P=0.000 01)。结论治疗原发性骨质疏松症方面,右归丸单用或与其它西药联合使用时优于单纯使用西药治疗,右归丸联合西药的疗效优于单纯右归丸,但由于研究纳入文献质量偏低,确切的研究结论仍然有待高质量的随机对照试验证实。
Objective This study is to evaluate the clinical efficacy of Modified Yougui Pill(右归丸) for osteoporosis through Meta-analysis.Method All literature published before January 7;, 2021 on randomized controlled trails of YouguiPill efficacy for osteoporosis were retrieved from 7 databases including China National Knowledge Infrastructure( CNKI),Wanfang Data,China Science and Technology Journal Database( CSTJ),China Biology Medicine disc( CBMdisc),Pub Med,Embase,and Cochrance Library. Cochrane risk assessment tool was adopted to assess the quality of the retrieved literature and Meta-analysis was undertaken with Rev Man5. 3. Results A total of 260 studies were retrieved,among which only 9 studies with 778 patients were included. Meta-analysis results were as follows. The total effective rate in Yougui Pill group was higher than that in the western medicine group( RR = 1. 25,95% CI: 1. 11-1. 40,P = 0. 000 1). The subgroup analysis showed that the total effective rate in the combining group of Yougui Pill and western medicine was higher than that in Yougui Pill group( RR = 1. 13,95% CI: 0. 90-1. 43). The bone density in Yougui Pill group was significantly higher than that in the western medicine group( MD =-1. 99,95% CI:-2. 33 ~-1. 65,P = 0. 000 01) and Osteocalcin in Yougui Pill group was lower than that in the western medicine group( MD =-1. 99,95% CI:-2. 33 ~-1. 65,P = 0. 000 01). Conclusion Yougui Pill,used alone or combined with western medicine,has higher clinical efficacy than western medicine alone in treating primary osteoporosis. The efficacy of Yougui Pill combined with western medicine is higher than Yougui Pill alone. But definite conclusions need to be drawn with higher level of evidence from multi-centre randomized controlled trails.
