摘要
目的:探讨放射治疗中应用靶区勾画MIM软件自动勾画宫颈癌危及器官(OAR)的精准度,探讨模板数据库病例数对勾画准确性的影响。方法:选取医院收治的70例宫颈癌患者,按照随机数表法在MIM软件中建立A数据库组(20例),在A数据库组的基础上再选取20例建立B数据库组(20+20例),在B数据库组的基础上再选取20例建立C组数据库(40+20例),数据库包含每例患者CT扫描图像和手工勾画图像资料;选取剩余10例宫颈癌患者的CT图像,分别使用3个数据库进行OAR自动勾画,与作为“金标准”的手工勾画图像进行比较。采用Dice相似性系数(DSC)、Hausdorff距离(HD)、Jaccard系数和体积差异性系数对MIM软件勾画OAR的精准度进行评价,并使用单因素方差分析判断模板数据库的病例数对自动勾画结果的影响。结果:在4项评价参数分析中,MIM软件对于脊髓、左股骨头、右股骨头、左肾以及右肾的自动勾画的DSC均>0.8,体积差异性系数<0.2;膀胱的DSC>0.8,但其余3项参数显示其自动勾画的结果欠佳;直肠的各项评价参数均未达到临床要求;不同组别间各个器官单因素方差分析结果无统计学差异。结论:应用MIM软件自动勾画宫颈癌OAR能较好地生成脊髓、左股骨头、右股骨头、左肾及右肾的图像轮廓,膀胱以及直肠的自动勾画未能满足临床要求;模板数据库的病例数对自动勾画OAR准确性无明显影响。
Objective:To explore the accuracy that MIM software of target volume delineation automatically delineated organ at risk(OAR)of cervical cancer in radiotherapy,and explore the influence of the number of cases of template database on the accuracy of delineation.Methods:70 patients with cervical cancer who admitted to hospital were selected.A database group(20 cases)was constructed in MIM software according to random number table,and other 20 cases were selected to construct B database group(20+20 cases)on the basis of A database group.And then,other 20 cases were further selected to construct C database group(40+20 cases)on the basis of B database group.The database contained computed tomography(CT)scan image and image material of manual delineation of each patient.The CT images of the last 10 cervical patients were automatically delineated for OAR by using 3 databases.And they were compared with the images of manual delineation which was used as gold standard.The Dice similarity coefficient(DSC),Hausdorff distance(HD),Jaccard coefficient and difference coefficient of volume were adopted to evaluate the accuracy of MIM software in delineating OAR.And the one-way ANOVA was used to determine the influence of the number of cases in template database on the results of automatic delineation.Results:In the analysis of 4 evaluation parameters,the all of DSCs of automatic delineation of MIM software were larger than 0.8 for spinal cord,left and right head of femur,and left and right kidney,and the difference coefficients of volume of that were smaller than 0.2 for those,and the DSC of bladder was larger than 0.8.While the other three evaluation parameters showed that the results of automatic delineation were poor.And each of evaluation parameters of rectum didn’t achieve the clinical requirements.The results of one-way ANOVA indicated there were no statistical significance for each organ between different groups.Conclusion:The application of MIM software in automatic delineation for cervical cancer OAR can better produce
