摘要
[目的]总结陶茂灿教授治疗成年女性斑秃的临床经验。[方法]通过跟师随诊,搜集陶教授临床治疗斑秃的案例并查阅文献,分析陶教授对斑秃病因病机的认识及从肝论治的具体方法,总结陶教授治疗斑秃的临床经验,并举案例一则加以佐证。[结果]陶教授认为成年女性斑秃与肝密切相关,总体可分为肝郁气滞型、肝火上炎型和肝血亏虚型,临床可采用疏肝理气、养血健脾、补肾填精为要点进行治疗,部分患者可出现兼证或夹杂症候,则需结合患者自身的特点辨证而论。所举案例中以此为要,根据患者临证表现,诊断为肝郁气滞之油风,治疗采用疏肝养血法,助发新生,疗效显著,后期加用补益肝肾、养心安神之中药巩固疗效。[结论]陶茂灿教授治疗成年女性斑秃重视从肝论治,疗效确切,其经验值得借鉴。
[Objective]To summarize Professor TAO Maocan’s treatment ideas and methods for adult female alopecia areata.[Methods]Through learning from teachers in outpatient service,collecting Professor TAO’s clinical treatment of alopecia areata cases and consulting the literature,Professor TAO’s understanding of the pathogenesis of alopecia areata and the specific methods of treatment from the liver were analyzed,the clinical experience in the treatment of alopecia areata was summarized,and a case was presented to support.[Results]Professor TAO believes that alopecia areata is closely related to the liver,which can be divided into three syndromes——liver depression and Qi stagnation syndrome,up-flaming of liver fire syndrome and liver blood deficiency syndrome.The clinical treatment can be based on the key points of soothing the liver and regulating Qi,nourishing blood and strengthening the spleen,invigorating the kidney and replenishing essence.Some patients may have mixed syndromes or mixed symptoms,the treatment should be based on their own characteristics.In the case cited,based on the clinical manifestations of the patient who got the alopecia areata,the symptoms was diagnosed as liver depression and Qi stagnation,the method of soothing the liver and nourishing blood was used to help regrow hair,and the curative effect was obvious.Later,the traditional Chinese medicine which used to tonifying the liver and kidney,nourishing the heart and tranquilizing mind was added to consolidate the curative effect.[Conclusion]Professor TAO treats adult female alopecia areata from liver which is effective and the experience is worth learning.
