摘要
目的评价顺蠕动侧-侧吻合在克罗恩病(CD)结肠部分切除吻合中的安全性和有效性。方法回顾性纳入及分析2017年1月至2020年9月期间因CD行顺蠕动侧-侧吻合的手术患者的临床及随访资料。结果共有6例CD患者纳入本研究,其中男性4例,女性2例。患者年龄为31.6±1.8(27~35)岁。平均手术时间为(155±74)min。无一例患者术后出现吻合口漏。术后中位住院时间为9(7~15)天。术后30天内无一例患者再次来本院、其他医院诊治或死亡。经过中位10(4~18)个月的随访,除1例患者因随访时间未满6个月,仅有1例患者出现内镜下复发(1/5)。无一例患者出现临床复发。结论顺蠕动侧-侧吻合适用于结肠部分切除的结肠CD的消化道重建,该吻合方式安全可行,可能具有降低吻合口漏等短期相关并发症以及降低远期吻合口复发的优势。
Objective This study aims to evaluate the safety and feasibility of side-to-side isoperistaltic anastomosis in segmental colectomy for Crohn's disease patients.Methods Clinical data,postoperative results and short-term outcomes of follow-up of enrolled CD patients,who underwent segmental colectomy and received side-to-side isoperistaltic anastomosis from January 2017 to December,2020 were retrospectively collected and analyzed.Results Totally six CD patients including 4 male and 2 female patients were enrolled,with an average age of(31.6±1.8)years old.The mean postoperative time was(155±74)min.No anastomotic leak occurred.Median postoperative time was 9(7~15)days.The rates of readmission,reoperation and mortality were all 0.After a median follow up of 10(4~18)months,only one endoscopic recurrence was observed among the 5 patients who had completed endoscopic surveillance endoscopic surveillance(one patient was followed up less than six months).There was no clinical or surgical recurrence.Conclusion The present study showed that side-to-side isoperistaltic anastomosis is feasible for CD patients who required segmental colectomy.Theoretically,this anastomotic technique has advantages both in reducing short-term complications,e.g.anastomotic leak,and long-term postoperative recurrence.
