摘要
目的:探讨腹腔镜下经胆囊管胆总管探查术治疗胆总管结石的效果及对免疫功能的影响。方法:选取本院2017年12月-2020年1月84例胆总管结石患者,按照随机数字表法分为对照组42例,采用常规开腹术治疗,观察组42例采用胆囊管胆总管探查术(腹腔镜下)治疗,观察两组围术期情况、疼痛状况、免疫功能及术后并发症。结果:观察组腹腔引流时间、住院时间均短于对照组,术中出血量少于对照组,手术时间长于对照组(P<0.05);术后3 d及术后1、2周VAS评分均低于对照组(P<0.05);术前术后,两组CD8^(+)、CD4^(+)、CD3^(+)及CD4^(+)/CD8^(+)水平对比差异无统计学意义(P>0.05);观察组并发症发生率(2.38%)低于对照组(19.04%),差异有统计学意义(P<0.05)。结论:腹腔镜下经胆囊管胆总管探查术治疗胆总管结石可显著改善围术期情况及疼痛状况,减少术后并发症的发生,且不影响免疫功能,但手术时间较长。
Objective:To investigate the effect of laparoscopic transcystic common bile duct exploration in the treatment of choledocholithiasis and its influence on immune function.Method:A total of 84 patients with choledocholithiasis in our hospital from December 2017 to January 2020 were divided into control group(42 cases)and observation group(42 cases)according to the random number table method.The patients in the control group were treated with conventional laparotomy,and the patients in the observation group were treated with transcystic common bile duct exploration(laparoscopic).The perioperative situation,pain status,immune function and postoperative complications of the two groups were observed.Result:The abdominal drainage time and hospitalization time of the observation group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,the operation time was longer than that of the control group(P<0.05);the VAS score at 3 d and 1 week and 2 weeks after surgery of the observation group were lower than those of the control group(P<0.05);the levels of CD8^(+),CD4^(+),CD3^(+)and CD4^(+)/CD8^(+)of the two groups before surgery and after surgery had no significant differences(P>0.05);the incidence of complications in the observation group(2.38%)was lower than that of the control group(19.04%),the difference was significant(P<0.05).Conclusion:Laparoscopic transcystic common bile duct exploration in the treatment of choledocholithiasis can significantly improve the perioperative situation,improve the pain of patients,and has less postoperative complications.It has no effect on immune function,but the operation time is longer.
