摘要
目的:利用3D打印技术为鼻腔NK/T细胞淋巴瘤患者定制个性化的组织补偿块,研究其在放疗计划中的剂量学影响。方法:选取10例早期鼻腔NK/T淋巴瘤患者,分别采用无组织补偿块的常规_(CTno-bolus)图像和3D打印组织补偿块的CT_(bolus)图像进行放疗计划设计,靶区处方剂量均为50Gy/25次。使用剂量体积直方图评估两种计划中计划靶区、危及器官的剂量学差异。结果:3D打印组织补偿块与体表紧密贴合,提高了靶区的均匀性和适形度;晶体、视神经和腮腺剂量在3D打印组织补偿块计划中均低于无组织补偿块计划(P<0.05)。结论:3D打印组织补偿块有效提高了鼻腔NK/T细胞淋巴瘤紧邻体表靶区的剂量分布,值得临床推广。
Objective To prepare a personalized bolus for patients with nasal natural killer(NT)/T-cell lymphoma using 3D printing technology,and to study the dosimetric effect of 3D printed bolus used for nasal NK/Tcell lymphoma in radiotherapy planning.Methods Ten patients with early nasal NK/T-cell lymphoma were enrolled in the study.CT images without bolus(CT_(no-bolus))and with 3D printed bolus(CT_(bolus))of each patient were acquired for treatment planning.The prescription dose of planning target volume(PTV)was 50 Gy/25 fractions.The dosimetric differences of PTV and organs-at-risk between two kinds of plans(Planbolus vs Planno-bolus)were evaluated using dose-volume histograms.Results The 3D printed bolus which closely fitted the body surface improved the uniformity and conformity of PTV.The doses to lens,optic nerves and parotids in Planbolus were all lower than those in Planno-bolus(P<0.05).Conclusion 3D printed bolus is worth to be promoted for clinic use for it can effectively improve the dose distribution of nasal NK/T-cell lymphoma target area adjacent to the epidermis.
