摘要
目的探讨经尿道前列腺电切术联合内分泌去势疗法治疗晚期前列腺癌的临床效果。方法选取2018年1月至2020年12月我院收治的50例晚期前列腺癌患者作为研究对象,依据患者入院编号的奇偶性将其随机分为对照组与观察组,各25例。对照组患者仅行内分泌去势治疗,观察组在对照组基础上联合经尿道前列腺电切术治疗。比较两组的临床疗效、生存质量及实验室指标。结果观察组治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组生理、心理、独立性、社会关系、环境、精神/宗教信仰领域评分均高于对照组,差异具有统计学意义(P<0.05)。治疗后2个月,观察组患者的IGF-1、PSA、VEGF水平低于对照组,残余尿量少于对照组,最大尿流率大于对照组,差异具有统计学意义(P<0.05)。结论经尿道前列腺电切术联合内分泌去势疗法治疗晚期前列腺癌患者的效果理想,可改善前列腺症状及排尿状态,提升患者的生存质量,具有重要临床应用价值。
Objective To investigate the clinical effect of transurethral resection of prostate combined with endocrine castration therapy in the treatment of advanced prostate cancer.Methods A total of 50 patients with advanced prostate cancer treated in our hospital from January 2018 to December 2020 were selected as the research objects.According to the parity of admission number,the patients were randomly divided into control group and observation group,with 25 cases in each group.The patients in the control group were only treated with endocrine castration,and the patients in the observation group were treated with transurethral resection of prostate on the basis of the control group.The clinical efficacy,quality of life and laboratory indexes were compared between the two groups.Results The total effective rate of treatment of the observation group was higher than that of the control group,and the difference was statisticaly significant(P<0.05).After treatment,the scores of physical,psychological,independence,social relationship,environment and spiritual/religious belief fields in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Two months after treatment,the levels of IGF-1,PSA and VEGF in the observation group were lower than those in the control group,the residual urine volume was less than that in the control group,and the maximum urinary flow rate was greater than that in the control group,and the differences were statistically significant(P<0.05).Conclusion Transurethral resection of prostate combined with endocrine castration therapy has an ideal effect in the treatment of patients with advanced prostate cancer.It can improve prostate symptoms,micturition status and the quality of life of patients,which has important clinical application value.
