摘要
目的探究左侧乳腺癌根治术后放疗中应用皮肤补偿膜对心功能的影响。方法44例行左侧乳腺癌根治术后放疗患者,遵循随机抽样原则分为对照组和治疗组,各22例。对照组行不加皮肤补偿膜的肿瘤调强计划,治疗组行全程加皮肤补偿膜的肿瘤调强计划。比较两组患者心电图改变结果及心肌酶改变情况。结果治疗组心率>100次/min、心电图T波改变、QT间期延长比例分别为9.1%、4.5%、0,均低于对照组的36.4%、27.3%、18.2%,差异有统计学意义(P<0.05);两组传导阻滞比例比较,差异无统计学意义(P>0.05)。治疗组心型肌酸激酶、超敏肌钙蛋白、血清肌红蛋白分别为(83.62±1.61)U/L、(0.08±0.00)μg/L、(52.93±5.85)ng/ml,均低于对照组的(126.42±3.02)U/L、(0.22±0.05)μg/L、(74.66±12.40)ng/ml,差异具有统计学意义(P<0.05)。结论左侧乳腺癌根治术后放疗中应用皮肤补偿膜对于降低心肌酶及心率、改善心电图异常率均具有显著的积极作用,值得在临床大力推广。
Objective To investigate the effect of skin compensatory film on cardiac function in radiotherapy after radical left mastectomy.Methods A total of 44 patients undergoing radiotherapy after radical left mastectomy were divided into control group and treatment group according to the principle of random sampling,with 22 cases in each group.The control group underwent tumor intensity modulation therapy without skin compensation film,and the treatment group underwent tumor intensity modulation therapy with skin compensation film.The changes of electrocardiogram and the changes of myocardial enzymes in the two groups were compared.Results The proportions of heart rate>100 beats/min,T-wave changes,QT interval prolongation in the treatment group were 9.1%,4.5%and 0,respectively,which were lower than 36.4%,27.3%and 18.2%in the control group,and the differences were statistically significant(P<0.05).The difference in the ratio of conduction block between the two groups was no statistically significant(P>0.05).The cardiac creatine kinase,hypersensitive troponin,and serum myoglobin in the treatment group were(83.62±1.61)U/L,(0.08±0.00)μg/L and(52.93±5.85)ng/ml,respectively,which were all lower than(126.42±3.02)U/L,(0.22±0.05)μg/L and(74.66±12.40)ng/ml in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of skin compensation film in radiotherapy after radical left mastectomy has a significant positive effect on reducing myocardial enzyme and heart rate,and improving abnormal rate of electrocardiogram,which is worth popularizing in clinic.
