摘要
目的探讨急性药物性肝损伤(acute drug-induced liver injury,ADILI)发生、发展过程中血清高迁移率族蛋白1(high mobility group protein B1,HMGB1)的动态变化及其与肝损伤指标的动态相关性研究。方法选用雄性SD大鼠随机分为对照组和模型组,模型组通过对乙酰氨基酚溶液灌胃法制备ADILI大鼠模型,模型制备成功后,动态检测不同时间点实验大鼠血清HMGB1、ALT、AST、ALP、TBIL、DBIL水平;对实验结果进行统计学分析。结果模型组血清HMGB1水平从3 h开始升高,24 h升至顶点,48 h开始下降,血清HMGB1水平曲线呈单峰形态,且0 h、3 h两个时间点血清HMGB1水平差异无统计学意义(P>0.05),12 h、24 h、48 h三个时间点之间血清HMGB1水平差异无统计学意义(P>0.05),48 h和96 h两个时间点之间血清HMGB1水平差异有统计学意义(P<0.05)。模型组在6 h、9 h、12 h、24 h、48 h、96 h 6个时间点血清HMGB1水平与对照组比较,差异有统计学意义(P<0.01),0 h、3 h时间点两者水平差异无统计学意义(P>0.05)。模型组血清HMGB1水平在9 h、12 h、24 h、48 h、96 h时与ALT、AST水平呈正相关(P<0.05),0 h、3 h时间点无相关性(P>0.05),模型组不同时间点血清HMGB1水平与ALP、TBIL、DBIL水平均无相关性(P>0.05)。结论ADILI模型血清HMGB1水平随着ADILI发生、发展呈单峰曲线动态变化,其水平变化与肝损伤指标AST、ALT水平变化呈正相关,与肝损伤指标ALP、TBIL、DBIL水平变化无相关性。
Objective To investigate the dynamic change of serum HMGB1 and its dynamic correlation with the liver injury index in the occurrence and development of acute drug-induced liver injury(ADILI).Methods Male SD rats were randomly divided into the control group and model group.The model group was prepared by intragastric administration of acetaminophen solution to prepare ADILI animal model.After the model was successfully prepared,the serum level of HMGB1,ALT,AST,ALP,TBIL and DBIL were dynamically detected at the different time points,then the experimental results were statistically analyzed.Results The serum HMGB1 level in model group increased at the 3rd hour,reached the peak at the 24th hour,then began to decline at the 48th hour.The serum HMGB1 level curve showed a single peak,in the meantime,there was no significant difference of the serum HMGB1 level between the begin and the 3rd hour(P>0.05),there was also no significant difference of the serum HMGB1 level among the 12th,24th and 48th hour points(P>0.05),and there was significant difference between 48th and 96th hour time points(P<0.05).The difference of the serum HMGB1 levels at 6th,9th,12th,24th,48th and 96th hour between the model group and the control group was statistically significant(P<0.01),while there was no statistically significant difference of the serum HMGB1 levels at the begin and the 3rd hour(P>0.05).The serum HMGB1 level in model group was positively correlated with ALT and AST levels at 9th,12th,24th,48th and 96th hour(P<0.05),but there was no correlation at the begin and the 3rd hour(P>0.05).There was no correlation of the serum HMGB1 level with the ALP,TBIL and DBIL levels at different time points in model group(P>0.05).Conclusion The serum HMGB1 level in ADILI model shows a single peak curve dynamic change in the occurrence and development of ADILI,its change has positively correlated with the level of AST and ALT,but has no correlation with the level of ALP,TBIL and DBIL.
