摘要
目的:对混合痔患者术后应用复方角菜酸酯乳膏涂抹联合自拟黄柏白及方保留灌肠的临床疗效进行观察。方法:纳入98例行混合痔手术患者为研究对象,按照随机数表法分为研究组与对照组各49例。对照组采用常规治疗,研究组在对照组基础上予以复方角菜酸酯乳膏涂抹联合自拟黄柏白及方保留灌肠治疗。记录治疗前及治疗2周后,两组患者中医症候(肛门肿痛、大便不畅、排便时出血)积分变化;比较两组患者术后疾病症状恢复(创面水肿消除时间、肛门胀痛刺激消失时间、创面出血消失时间)评估差异;分析治疗前及治疗2周后,两组患者肛门动力指标[直肠静息压(PRR)、肛管静息压(PAR)、肛管收缩压(PAS)]、血清指标[血小板源性生长因子(PDFG)、血管内皮生长因子(VEGF)、碱性成纤维生长因子(bFGF)]水平变化。结果:治疗2周后,两组患者肛门肿痛、大便不畅、排便时出血评分较治疗前降低(P<0.05),且研究组低于同一时间对照组(P<0.05);研究组患者创面水肿消除时间、肛门胀痛刺激消失时间、创面出血消失时间短于对照组(P<0.05);治疗2周后,研究组患者血清PRR、PAR、PAS水平均高于治疗前(P<0.05),且高于同一时间对照组(P<0.05);治疗2周后,两组患者PDFG、VEGF、bFGF水平均高于治疗前(P<0.05),且研究组高于同一时间对照组(P<0.05)。结论:复方角菜酸酯乳膏涂抹联合自拟黄柏白及方保留灌肠应用于混合痔术后,有助于促进患者疾病恢复,改善其肛肠动力状态,纠正血清指标水平。
Objective:To observe the clinical efficacy of postoperative application of Compound Carraghenates Cream combined with Self-made Huangbai Baiji Prescription retention enema for patients with mixed hemorrhoids.Methods:A total of 98 patients undergoing mixed hemorrhoid surgery were enrolled as the research subjects.According to the random number table method,they were divided into a study group and a controlled group,with 49 cases in each group.The controlled group was given conventional treatment,and the study group was given Compound Carraghenates Cream combined with Self-made Huangbai Baiji Prescription retention enema therapy on the basis of the controlled group.Before treatment and after 2 weeks of treatment,the scores of TCM symptoms(anal swelling and pain,poor bowel movements,bleeding during defecation)were recorded in the two groups.The recovery of postoperative disease symptoms(elimination time of wound edema,disappearance time of analswelling and pain stimulation,disappearance time of wound bleeding)were compared between the two groups.Before treatment and after 2 weeks of treatment,the anal motility indexes[rectal resting pressure(PRR),anal resting pressure(PAR),anal systolic pressure(PAS)]and serum indexes[platelet-derived growth factor(PDFG),vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)]were analyzed in the two groups.Results:After 2 weeks of treatment,the scores of anal swelling and pain,bowel movements and bleeding during defecation in the two groups were lower than those before treatment(P<0.05),and the scores in the study group were lower than those in the controlled group at the same time point(P<0.05).The elimination time of wound edema,disappearance time of anal swelling and pain stimulation,and disappearance time of wound bleeding in study group were shorter than those in the controlled group(P<0.05).After 2 weeks of treatment,the levels of serum PRR,PAR and PAS in the study group were higher than those before treatment(P<0.05),and were higher than those in the co
