去白细胞红细胞输血对严重创伤患者免疫功能和炎症因子的影响 被引量:4

Effects of Erythrocyte Deleukocyte Transfusion on Immune Function and Inflammatory Factors in Patients with Severe Trauma

摘要 目的探析严重创伤患者应用去白细胞红细胞输血对免疫功能与炎症因子的影响。方法选取2018年6月—2020年6月在该院治疗的严重创伤患者60例为研究对象,按照随机抽签法分组,组别为参照组与研究组。参照组30例患者予以输悬浮红细胞,研究组30例患者予以去白细胞红细胞输血,对比两组输血前后免疫功能与炎症因子水平变化。结果研究组输血后CD3+、CD4+、CD8+、CD4+/CD8+分别为(47.45±6.56)%、(22.56±4.56)%、(22.89±4.78)%、(0.98±0.12),明显高于参照组,差异有统计学意义(t=2.805、5.175、3.812、2.355,P<0.05)。研究组输血后1 d白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平分别为(386.78±32.13)、(5.05±1.08)pg/mL,与参照组比较差异无统计学意义(t=0.407、0.404,P>0.05);研究组输血后7 d的IL-6、TNF-α水平分别为(263.45±26.67)、(3.65±0.76)pg/mL,明显高于参照组,差异有统计学意义(t=14.119、9.324,P<0.001)。结论严重创伤患者予以去白细胞红细胞输血,能够有效延缓炎症因子水平下降,有助于患者康复,值得临床推荐应用。 Objective To investigate the effects of blood transfusion on immune function and inflammatory factors in patients with severe trauma.Methods A total of 60 patients with severe trauma treated in the hospital were selected from June 2018 to June 2020 as study subjects.The patients were divided into the reference group and the study group according to the random lottery method.30 patients in the reference group were given suspended red blood cell transfusion,and 30 patients in the study group were given RBC deleukocyte transfusion.The changes of immune function and inflammatory factors before and after blood transfusion were compared between the two groups.Results After transfusion,CD3+,CD4+,CD8+and CD4+/CD8+in the study group were(47.45±6.56)%,(22.56±4.56)%,(22.89±4.78)%and(0.98±0.12),respectively,which were significantly higher than those in the reference group,and the differences were statistically significant(t=2.805,5.175,3.812,2.355,P<0.05).The levels of interleukin-6(IL-6)and tumor necrosis factor(TNF-a)in the study group were(386.78±32.13)pg/mL and(5.05±1.08)pg/mL,respectively,1 d after transfusion,there was no statistically significant difference compared with the reference group(t=0.407,0.404,P>0.05).The levels of IL-6 and TNF-in the study group were(263.45±26.67)pg/mL and(3.65±0.76)pg/mL,respectively,7 days after transfusion,which were significantly higher than those in the reference group,the difference was statistically significant(t=14.119,9.324,P<0.001).Conclusion Transfusion of deleukocyte red blood cells in patients with severe trauma can effectively delay the decline of inflammatory factors and contribute to the recovery of patients,which is worthy of clinical recommendation.
