摘要
目的了解青海省班玛县人群棘球蚴病感染情况及防治知识知晓情况。方法 2017—2018年采用便携式B超仪对班玛县受检者腹腔脏器进行扫描,依据《包虫病诊断标准》、流行病学史及临床表现予以诊断,对当地重点人群进行棘球蚴病知晓情况调查,内容包括防治知识、态度及日常行为。结果影像学检查28 568人,检出棘球蚴病患者388例,检出率1.36%。不同性别、年龄、居住方式、职业、文化程度、地区间人群棘球蚴病检出率差异均有统计学意义(χ^(2)=38.07、166.766、111.691、186.339、81.049、232.562,P均<0.01)。2017年棘球蚴病知晓率91.25%(386/423),2018年为94.88%(408/430),差异有统计学意义(χ^(2)=4.366,P=0.037)。2017年与2018年,不同职业(χ^(2)=14.085、14.047,P=0.015、0.015)及不同文化程度(χ^(2)=21.480、9.578,P<0.01、0.048)人群棘球蚴病防治知识知晓率差异均有统计学意义。结论班玛县棘球蚴病防治效果较好,但棘球蚴病防治形势依然严峻,今后仍需进一步加强以重点人群为主的教育宣传工作,加大棘球蚴病防治力度。
Objective To understand the prevalence of human Echinococcosis and knowledge awareness for the disease in Banma county of Qinghai province. Methods Local residents aged over 2 years in Banma county were randomly recruited in the study.Portable B-ultrasound instrument was used to scan abdominal organs of participants.The diagnosis of Echinococcosis was made according to the Diagnostic Criteria for Echinococcosis combined with epidemiological history and clinical manifestations.Meanwhile, a questionnaire survey was conducted to collected disease awareness-related information, including knowledge of Echinococcosis prevention and control, attitude and daily behavior of participants. Results A total of 28 568 residents received B-ultrasound examination, and 388 Echinococcosis cases were found, with the overall prevalence of 1.36%.There were statistically significant differences in the prevalence rate of Echinococcosis among residents with different gender, age, occupation, education, residing pattern and residency(χ^(2)=38.07, 166.766, 111.691, 186.339,81.049, 232.562, P<0.01).The awareness rate for Echinococcosis prevention and control was 91.25%(386/423) in 2017 and 94.88%(408/430) in 2018, the difference was statistically significant(χ^(2)=4.366, P=0.037).In both 2017 and 2018, the awareness rates varied significantly among residents with different occupations(χ^(2)=14.085, 14.047, P=0.015,0.015)and education levels(χ^(2)=21.480, 9.578, P<0.01, 0.048). Conclusions The overall awareness rate for Echinococcosis prevention and control among local residents in Banma county is relatively high,which may be attributable to the lower prevalence rate of Echinococcosis in the area.In order to further decrease the parasite infection rate,health education should be strengthened,especially among residents with higher risk of infection.
