摘要
目的探讨可溶性程序性死亡受体-1(sPD-1)、可溶性程序性死亡配体-1(sPD-L1)和γ-干扰素(IFN-γ)在晚期肿瘤患者血清中的表达特征及其临床意义。方法检测晚期肺癌患者(肺癌组)和消化道肿瘤患者(消化道肿瘤组)血清免疫标志物sPD-1、sPD-L1、IFN-γ和肿瘤标志物甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原153(CA153)、糖类抗原199(CA199)、糖类抗原724(CA724)、细胞角蛋白19片段(Cyfra211)、神经元特异性烯醇化酶(NSE)的水平,分析其与健康人(体检组)和肺炎患者(肺炎组)间的差异;分析肿瘤患者IFN-γ及上述肿瘤标志物对sPD-1、sPD-L1的影响;分析sPD-1与sPD-L1,sPD-L1与IFN-γ的相关性。结果消化道肿瘤组和肺癌组CEA水平高于肺炎组和体检组(P<0.01);消化道肿瘤组CA125水平高于肺炎组(P<0.05);肺癌组CA153水平高于肺炎组(P<0.05);消化道肿瘤组CA199水平高于体检组(P<0.01);消化道肿瘤组CA724水平高于肺炎组(P<0.05);肺癌组、消化道肿瘤组Cyfra211水平高于体检组和肺炎组(P<0.01);肺癌组NSE水平高于体检组(P<0.05);肿瘤患者sPD-1和sPD-L1水平相对肺炎患者和体检者有升高趋势,其中消化道肿瘤组sPD-1水平高于体检组(P<0.05);消化道肿瘤组sPD-L1/IFN-γ高于肺炎组(P<0.01)。不同水平IFN-γ的患者其sPD-1和sPD-L1分布差异有统计学意义(P<0.01);sPD-1与sPD-L1在肺癌患者、消化道肿瘤患者和全部肿瘤患者中均呈正相关(r=0.688、0.594、0.550,P<0.05);在肺炎患者中IFN-γ与sPD-L1呈正相关(r=0.547,P<0.05)。结论sPD-1与sPD-L1在晚期肿瘤患者外周循环中整体呈高表达并且具有同步变化趋势;IFN-γ水平会影响sPD-1和sPD-L1的分布,sPD-1、sPD-L1和IFN-γ的表达特征对反映晚期肿瘤的免疫状态具有重要参考价值,在肿瘤的临床诊疗中具有潜在的生物标志物价值。
Objective To investigate the expression and clinical significance of of soluble programmed death-1(sPD-1),soluble programmed death ligand-1(sPD-L1)and interferon-γ(IFN-γ)in serum of patients with advanced cancer.Methods The serum levels of sPD-1,sPD-L1,IFN-γand tumor markers such as alpha fetoprotein(AFP),carcinoembryonic antigen(CEA),carbohydrate antigen(CA)125,CA153,CA199,CA724,cytokeratin 19 fragments(Cyfra211)and neuron specific enolase(NSE)were detected in patients with advanced lung cancer(lung cancer group)and gastrointestinal cancer(gastrointestinal cancer group),the differences between them and healthy persons(physical examination group)and pneumonia patients(pneumonia group)were analyzed;the effects of IFN-γand above tumor markers on sPD-1 and sPD-L1 in tumor patients were analyzed;the correlation between sPD-1 and sPD-L1,sPD-L1 and IFN-γwas analyzed.Results The level of CEA in gastrointestinal tumor group and lung cancer group was higher than that in pneumonia group and physical examination group(P<0.01).The level of CA125 in gastrointestinal tumor group was higher than that in pneumonia group(P<0.05).The level of CA153 in lung cancer group was higher than that in pneumonia group(P<0.05).The level of CA199 in gastrointestinal tumor group was higher than that in physical examination group(P<0.01).The level of CA724 in gastrointestinal tumor group was higher than that in pneumonia group(P<0.05).The level of Cyfra211 in lung cancer group and gastrointestinal tumor group was higher than that in physical examination group and pneumonia group(P<0.01).The level of NSE in lung cancer group was higher than that in physical examination group(P<0.05).The levels of sPD-1 and sPD-L1 in tumor patients were higher than those in pneumonia patients and physical examination people,and the level of sPD-1 in gastrointestinal tumor group were higher than that in physical examination group(P<0.05).The sPD-L1/IFN-γin gastrointestinal tumor group was higher than that in pneumonia group(P<0.01).The distribution of sPD-1
