摘要
目的 探讨2型糖尿病(T2DM)患者胰岛素抗体与血尿酸(SUA)、甲状腺功能的相关性。方法 选取150例T2DM患者作为观察组,另选取150例健康体检者作为对照组。分析两组一般资料及胰岛素抵抗指数(HOMA-IR)、SUA、甲状腺功能等实验室指标;根据T2DM患者HOMA-IR采用四分位法进行分组,在1.85、3.01、4.83三个切点值下分为四组,0.43~1.85为Q1组(28例),1.85~3.01为Q2组(43例),3.01~4.83为Q3组(47例),4.83以上为Q4组(32例)。比较观察组和对照组的年龄、体质量指数(BMI)、HOMA-IR、SUA、总三碘甲状腺原氨酸(TT3)、总甲状腺素(TT4)、促甲状腺素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4);比较Q1、Q2、Q3、Q4组的年龄、病程、BMI、SUA、TT3、TT4、TSH、FT3、FT4;分析胰岛素抗体与SUA、甲状腺功能的相关性。结果观察组与对照组年龄、BMI、TT4、FT3及FT4水平比较,差异无统计学意义(P>0.05);观察组HOMA-IR(3.98±1.32)、SUA(305.43±48.62)μmol/L、TSH(1.44±0.58)mU/L均高于对照组的(2.79±0.78)、(264.19±31.87)μmol/L、(1.17±0.43)mU/L,TT3(1.87±0.26)nmol/L低于对照组的(1.98±0.23)nmol/L,差异有统计学意义(P<0.05)。Q1组到Q4组,随着HOMA-IR升高,BMI、SUA、TSH水平逐渐上升,TT3水平逐渐降低;Q2组、Q3组及Q4组的BMI、SUA均高于Q1组,差异有统计学意义(P<0.05);Q3组及Q4组的TSH水平高于Q1组,TT3水平低于Q1组,差异有统计学意义(P<0.05);Q4组BMI、SUA、TSH水平均高于Q2组,TT3水平低于Q2组,差异有统计学意义(P<0.05)。Pearson相关性分析显示,HOMA-IR与BMI、SUA、TSH呈正相关(r=0.719、0.675、0.538,P=0.001、0.003、0.014<0.05),与TT3呈负相关(r=-0.492,P=0.029<0.05)。结论 T2DM患者的胰岛素抗体水平升高与高SUA、甲状腺功能异常有关,SUA、TT3、TSH水平变化是T2DM患者胰岛素抗体形成和发展的危险因素。
Objective To discuss the correlation between insulin antibody and serum uric acid(SUA)and thyroid function in patients with type 2 diabetes mellitus(T2DM).Methods 150 cases of T2DM patients were selected as the observation group,and 150 healthy subjects were selected as the control group.The general data and laboratory indicators such as homeostasis model assessment-insulin resistant index(HOMA-IR),SUA,and thyroid function of the two groups were analyzed.Patients with T2DM were grouped according to HOMA-IR using the quartile method and divided into four groups under three cut-off values of 1.85,3.01,and 4.83,with 0.43-1.85 as Q1 group(28 cases),1.85-3.01 as Q2 group(43 cases),3.01-4.83 as Q3 group(47 cases),and>4.83 as Q4 group(32 cases).Comparison was made on age,body mass index(BMI),HOMA-IR,SUA,total triiodothyronine(TT3),total thyroxine(TT4),thyroid stimulating hormone(TSH),free triiodothyronine(FT3),free tetraiodothyronine(FT4)between the observation group and control group.The age,disease course,BMI,SUA,TT3,TT4,TSH,FT3,FT4 of the Q1,Q2,Q3,and Q4 groups were compared,and the correlation between insulin antibodies and SUA and thyroid function were analyzed.Results There was no statistically significant difference in age,BMI,TT4,FT3 and FT4 levels between the observation group and the control group(P>0.05).HOMA-IR(3.98±1.32),SUA(305.43±48.62)μmol/L,and TSH(1.44±0.58)mU/L of the observation group were higher than(2.79±0.78),(264.19±31.87)μmol/L,and(1.17±0.43)mU/L of the control group,and TT3(1.87±0.26)nmol/L was lower than(1.98±0.23)nmol/L of the control group.All the differences were statistically significant(P<0.05).Q1 group to Q4 group,as HOMA-IR increased,BMI,SUA and TSH levels gradually increased and TT3 levels gradually decreased.BMI and SUA in the Q2,Q3 and Q4 groups were higher than those in the Q1 group,and the differences were statistically significant(P<0.05).TSH levels in the Q3 and Q4 groups were higher than those in the Q1 group,and TT3 levels were lower than those in the Q1 group.All the
