数字化胃肠造影在小儿胃肠道梗阻性疾病中的诊断及应用价值

Analysis of the diagnostic status and utility of digital gastroenterography in pediatric patients with intestinal obstructive disease

摘要 目的探讨小儿胃肠道梗阻性疾病诊断中使用数字化胃肠造影的价值。方法纳入50例小儿胃肠道梗阻性疾病患者进行研究(2020年1月至2021年3月),均进行数字化胃肠造影,以手术病理结果为金标准,分析2种方案检出率、符合率。结果(1)检出率:数字化胃肠造影检出11例先天性肥厚性幽门狭窄(CHPS),占22%;胃食管反流(GER)10例,占20%;十二指肠梗阻6例,占12%;肠重复畸形7例,占14%;肠套叠7例,占14%;低位小肠或大肠梗阻7例,占14%,检出率(96%)与金标准(100%)对比差异无统计学意义(P>0.05)。(2)符合率:数字化胃肠造影与金标准检查CHPS符合率91.7%、GER符合率100%、十二指肠梗阻符合率100%、肠重复畸形符合率87.5%、肠套叠符合率100%、低位小肠或大肠梗阻符合率100%,总符合率为96%。结论数字化胃肠造影在小儿胃肠道梗阻性疾病诊断中检出率较高,且该方案与金标准符合率较高,并具有无创、可重复操作等优势,可为后期治疗提供参考。 Objective To investigate the value of digital gastroenterography in the diagnosis of pediatric gastrointestinal obstructive diseases.Methods Included in this study were 50 pediatric patients with obstructive disease of the gastrointestinal tract(between January 2020 and March 2021).Digital gastroenterography was applied to all 50 patients.The surgical pathology results were used as the significant standard to analyze the detection and coincidence rates of the two protocols.Results(1)Detection rates:11 cases of congenital hypertrophicpyloric stenosis(CHPS)(22%),10 cases of gastroesophageal reflux(GER)(20%),6 cases of duodenum obstruction(12%),7 cases of intestinal duplication(14%),7 cases of intussusception(14%)and 7 cases of low intestinal or large intestinal obstruction(14%)were detected by digital gastroenterography.There was no statistically significant difference between detection rate(96%)and pathological diagnosis(P>0.05).(2)Coincidence rates:The coincidence rates of CHPS,GER,duodenum obstruction,intestinal duplication,intussusception and low intestinal or large intestinal obstruction were 91.7%,100%,87.5%,100%and 100%respectively and the total coincidence rate was 96%.Conclusion Digital gastroenterography has a high detection rate in the diagnosis of pediatric gastrointestinal obstructive diseases,and this protocol has a high coincidence rate.Digital gastroenterography is noninvasive and reproducible and it can also provide a reference for later treatment.
