摘要
目的探讨针刀联合针刺治疗早期膝骨性关节炎急性期的临床疗效。方法选取2020年3月至2021年3月就诊于福建省立医院康复二科的62例早期膝骨性关节炎急性期患者,按随机数字表随机分为对照组、治疗组各31例,对照组采用常规针刺方法,治疗组在常规针刺治疗的基础上配合针刀松解腰骶部痉挛肌群,并于治疗前、治疗3个疗程结束后,分别记录两组的疼痛视觉模拟评分(visual analogue scale,VAS)和Lequesne指数评分,并作统计分析,以评估疗效。结果干预周期结束后,两组患者的VAS评分、Lequesne指数评分均低于治疗前,差异具有统计学意义(治疗组治疗前后VAS评分Z=﹣4.923,Lequesne指数评分Z=﹣4.896;对照组治疗前后VAS评分Z=﹣4.949,Lequesne指数评分Z=﹣4.871,P均<0.05);进一步比较发现,治疗组治疗后的VAS评分、Lequesne指数评分均低于对照组治疗后评分,差异具有统计学意义(VAS评分Z=3.659,Lequesne指数评分Z=3.984,P均<0.05)。结论针刀联合针刺治疗早期膝骨性关节炎急性期可有效缓解症状,改善膝关节功能,适合临床推广使用。
Objective To investigate the clinical effectiveness of acupotomy combined with acupuncture in the treatment of mild knee osteoarthritis at acute stage.Methods According to the random number table,62 cases of patients with mild knee osteoarthritis at acute stage were randomly divided into control group and treatment group.Thirty-one cases in control group received acupuncture treatment,while other 31 cases in treatment group underwent acupotomy to release the spasm of lumbar and sacral muscle groups based on the therapy in control group.Before and after 3 courses of treatment,visual analogue scale(VAS)and Lequesne index were recorded respectively in the two groups,and then statistical analysis was conducted to evaluate the curative effectiveness.Results After 3 courses of treatment,VAS score and Lequesne index of patients in the treatment group were lower than those in the control group(VAS score Z=3.659,Lequesne index Z=3.984,P<0.05).Conclusion The symptoms on mild knee osteoarthritis at acute stage can be effectively relieved by acupotomy combined with acupuncture treatment,and it can also improve the function of knee joint and prevent the development of knee osteoarthritis.
