摘要
目的评价微创拔牙术联合Er:YAG激光术后创面处理在埋伏阻生齿中的应用效果。方法选取2018年1月-2019年6月河北省胸科医院口腔科就诊的100例埋伏阻生牙患者,随机分为两组,每组50例。两组均采用微创拔牙术,对照组术后常规处理创面,观察组术后以Er:YAG激光处理创面。对比两组术后即刻、术后3 d、术后7 d的创面恢复效果、疼痛情况及龈沟液中炎症介质变化,统计术后并发症发生率。结果观察组术后即刻、术后3 d创面愈合良好率高于对照组(P<0.05);两组术后7 d创面愈合良好率比较,差异无统计学意义(P>0.05)。与术后即刻比较,两组术后3 d VAS评分均升高(P<0.05);与术后3 d比较,两组术后7 d VAS评分均下降(P<0.05);观察组术后3 d VAS评分低于对照组(P<0.05)。与术后即刻比较,两组术后3 d、术后7 d肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)水平更低(P<0.05);与术后3 d比较,两组术后7 d TNF-α、IL-1β、IL-6水平更低(P<0.05);观察组术后即刻、术后3 d TNF-α、IL-1β、IL-6水平均低于对照组(P<0.05)。观察组术后并发症发生率低于对照组(P<0.05)。结论微创拔牙术联合Er:YAG激光术后创面处理应用于埋伏阻生齿中,更利于促进创面愈合,减轻疼痛及炎症反应,且并发症发生率低,具有临床推广价值。
Objective To evaluate the application of minimally invasive extraction combined with Er:YAG laser in the treatment of impacted teeth.Methods A total of 100 patients with impacted teeth in Hebei provincial Chest Hospital from January 2018 to June 2019 were randomly divided into two groups,50 cases in each group.Both groups were treated with minimally invasive tooth extraction,the control group was treated with conventional wound treatment,and the observation group was treated with Er:YAG laser.The effect of wound recovery,pain conditions and changes in inflammatory mediators in gingival crevicular fluid were compared between the two groups immediately after surgery,3 days after surgery,and 7 days after surgery,and the incidence of postoperative complications was calculated.Results The wound healing rate of the observation group immediately and 3 days after the operation was higher than that of the control group(P<0.05).There was no significant difference in the wound healing rate between the two groups at 7 days postoperatively(P>0.05).Compared with immediately after operation,the VAS scores of the two groups increased at 3 days after the operation(P<0.05).Compared with 3 days after operation,the VAS scores of the two groups decreased at 7 days after operation(P<0.05).The VAS score of the observation group was lower than that of the control group at 3 days after operation(P<0.05).Compared with immediately after operation,the levels of TNF-α,IL-1βand IL-6 were lower in the two groups at 3 and 7 days after operation(P<0.05).Compared with 3 days after operation,the levels of TNF-α,IL-1βand IL-6 were lower in the two groups at 7 days after operation(P<0.05).The levels of TNF-α,IL-1βand IL-6 in the observation group immediately after operation and 3 days after operation were lower than those in the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion Minimally invasive tooth extraction combined with Er:YAG laser pos
