床旁超声对创伤休克患者下腔静脉管径变化的监测效果研究 被引量:3

摘要 目的:通过使用床旁超声仪监测,分析创伤失血性休克患者下腔静脉管径变化。方法:记录2019年8月至2020年10月期间收治于我院的创伤失血性休克患者49例,经床旁超声仪分别测量记录创伤患者失血性休克及经治疗纠正休克后的下腔静脉管径数据,根据其结果计算评估患者下腔静脉内径形变指数(shape change index,SCI)。结果:复苏后患者下腔静脉内径形变指数(SCI)较前减低,前后对比差异具有统计学意义(P<0.05)。结论:在创伤失血患者休克状态监测过程中,通过床旁超声仪测量下腔静脉内径可评估患者循环衰竭情况,为快速诊断提供有效依据,及时给予患者正确病情评估及治疗。 Objective:The changes in the diameter of the inferior vena cava in patients with traumatic hemorrhagic shock were analyzed by using bedside ultrasound equipment.Methods:Collected 49 patients with traumatic hemorrhagic shock admitted to our hospital from August 2019 to October 2020,measured and recorded the data of the inferior vena cava diameter of trauma patients with hemorrhagic shock and shock corrected by treatment by bedside ultrasound instrument.Calculate and evaluate the shape change index(SCI)of the inferior vena cava based on the results,and analyze and discuss the changes in the diameter of the inferior vena cava in patients with traumatic hemorrhagic shock.Results:After resuscitation,the maximum and minimum values of the inner diameter of the inferior vena cava became smaller,and the inner diameter of the inferior vena cava index(SCI)was lower than before.The difference between before and after was statistically significant(P<0.05).Conclusion:In the diagnosis and treatment of trauma and hemorrhagic shock,measuring the inner diameter of the inferior vena cava by bedside ultrasound can monitor the patient's circulating blood volume and assess the patient's blood volume.It can provide an effective basis for the rapid diagnosis of traumatic hemorrhagic shock.During the fluid resuscitation treatment of shock correction,the effect of fluid resuscitation can be effectively monitored,and the fluid resuscitation treatment plan can be adjusted in time to provide a basis for the diagnosis and treatment process of patients with traumatic hemorrhagic shock,improve the viability of this type of patients,and improve the prognosis.
