摘要
目的:分析翼状胬肉切除术(PE)联合颞上方角膜缘干细胞移植术(ALSCT)对翼状胬肉患者术后恢复及眼部功能的影响。方法:选择某院收治的翼状胬肉患者66例,以信封法随机分为对照组(33例)和观察组(33例),对照组接受PE治疗,观察组采用PE联合ALSCT治疗,比较2组患者临床疗效、创面愈合时间、手术前后泪膜功能[泪膜破裂时间(BUT)、基础泪液分泌试验(SIT)]、视觉功能[角膜散光度(CAD)、裸眼视力(UCVA)、角膜曲率]、不良反应及复发情况。结果:观察组治疗有效率为96.97%,高于对照组的72.73%,观察组创面愈合时间短于对照组(P值均<0.05);治疗后2组患者BUT、SIT水平均较治疗前上升,且观察组高于对照组(P<0.05);治疗后2组患者UCVA和角膜曲率明显升高,CAD降低,且观察组UCVA及角膜曲率高于对照组,CAD低于对照组(P值均<0.05);观察组不良反应发生率及复发率低于对照组(P<0.05)。结论:PE联合ALSCT治疗翼状胬肉疗效显著,可提高患者泪膜功能和视力功能,改善角膜散光,加快创面愈合,且不良反应发生和复发率低,具有临床应用价值。
Objective:To analyze the effects of pterygium excision(PE)combined with superior temporal limbal stem cell transplantation(ALSCT)on postoperative recovery and ocular function of patients with pterygium.Methods:A total of 66 patients with pterygium admitted to a hospital were selected and were randomly divided into a control group(33 cases)and an observation group(33 cases)by envelope method.The control group was treated with PE,and the observation group underwent PE combined with ALSCT.Clinical efficacy,wound healing time,tear film function[tear film rupture time(BUT),basic tear secretion test(SIT)]before and after surgery,visual function[corneal astigmatism(CAD),uncorrected visual acuity(UCVA),corneal curvature],adverse reactions and recurrence were compared between the two groups.Results:After the treatment,the effective rate of the observation group was 96.97%,significantly higher than 72.73%of the control group(P<0.05).The wound healing time of the observation group was(3.92±1.18)d,shorter than(5.85±1.20)d of the control group(P<0.05).After the treatment,the levels of BUT and SIT increased in both groups(P<0.05),and the observation group was higher(P<0.05);After the treatment,UCVA and corneal curvature of the two groups increased,while CAD decreased(P<0.05),and UCVA and corneal curvature of the observation group were higher,while CAD was lower(P<0.05).The incidence of adverse reactions and recurrence rate were lower in the observation group(P<0.05).Conclusion:PE combined with ALSCT is effective in the treatment of pterygium,which can ameliorate the tear film function and visual function,improve corneal astigmatism,accelerate wound healing,and has low incidence of adverse reactions and recurrence rate.
