机械通气治疗ICU严重多发伤患者的临床观察 被引量:2

摘要 目的评价机械通气治疗重症加强护理病房(ICU)严重多发伤患者的临床疗效,为ICU治疗提供参考。方法80例ICU诊治的严重多发伤患者,随机分为对照组与观察组,各40例。对照组患者采用常规治疗,观察组患者采用早期机械通气治疗。比较两组患者并发症发生情况、血气指标水平。结果观察组患者的脓毒症、器官衰竭、肺部感染、死亡发生率分别为5.00%、5.00%、2.50%、2.50%,均明显低于对照组的25.00%、25.00%、20.00%、15.00%,差异具有统计学意义(P<0.05)。观察组患者的酸碱度(pH)值(7.38±0.06)、血氧分压(81.50±5.50)mm Hg(1 mm Hg=0.133 kPa)、血碳酸氢根(23.90±1.50)mmol/L、血氧饱和度(94.50±5.50)%均明显高于对照组的(7.20±0.05)、(42.60±3.30)mm Hg、(21.05±1.30)mmol/L、(72.60±4.50)%,动脉血二氧化碳分压(36.60±2.05)mm Hg明显低于对照组的(40.20±2.50)mm Hg,差异具有统计学意义(P<0.05)。结论ICU严重多发伤患者具有较高的致死风险,早期机械通气治疗效果较为理想,可以有效改善患者的血气指标,维持患者正常呼吸功能,进而提高患者的生存率、降低感染等相关并发症风险。 Objective To evaluate the clinical efficacy of mechanical ventilation therapy on patients with severe multiple injuries in intenseive care unit(ICU),so as to provide references for ICU treatment.Methods A total of 80 ICU patients with severe multiple injuries were randomly divided into a control group and an observation group,with 40 cases in each group.The control group was treated with conventional therapy,and the observation group was treated with early mechanical ventilation.The complications and blood gas index levels were compared between the two groups.Results The incidence of sepsis,organ failure,pulmonary infection,and death in the observation group was 5.00%,5.00%,2.50%,and 2.50%,respectively,all of which were significantly lower than 25.00%,25.00%,20.00%,and 15.00%in the control group,and the differences were statistically significant(P<0.05).The pH(7.38±0.06),partial pressure of blood oxygen(81.50±5.50)mm Hg(1 mm Hg=0.133 kPa),blood bicarbonate(23.90±1.50)mmol/L,and oxygen saturation(94.50±5.50)%in the observation group were significantly higher than(7.20±0.05),(42.60±3.30)mm Hg,(21.05±1.30)mmol/L,(72.60±4.50)%in the control group,and arterial partial pressure of carbon dioxide(36.60±2.05)mm Hg was significantly lower than(40.20±2.50)mm Hg in the control group.All the difference was statistically significant(P<0.05).Conclusion Patients with severe multiple injuries in the ICU have a higher risk of death.Early mechanical ventilation has an ideal effect.It can effectively improve the patient’s blood gas indicators and maintain the patient’s normal respiratory function,thereby increasing the patient’s survival rate and reducing the risk of infection and other related complications.
