摘要
目的探究与分析肉眼血尿标本中红细胞对白细胞酯酶的影响及处理措施。方法选取2020年1月—2021年1月本院收集的肉眼血尿标本100份,采用尿干化学分析仪检测尿标本中的白细胞酯酶水平,之后对血尿标本采用离心机做离心处理,再次对白细胞酯酶水平采用尿干化学分析仪检测,同时将传统的手工沉渣镜检结果作为金标准,对比血尿标本实施离心处理前后,采用尿干化学分析仪检测后的白细胞酯酶的假阳性率。同时分析不同红细胞浓度与白细胞酯酶检测阳性及阴性的关系。结果肉眼血尿标本中未离心前白细胞酯酶假阳性率为94.81%。肉眼血尿标本行离心后白细胞酯酶假阳性率为32.47%。肉眼血尿标本行离心处理后白细胞酯酶假阳性率低于离心处理前,差异有统计学意义(P<0.05)。分析不同红细胞浓度与白细胞酯酶检测阳性及阴性的关系时发现在肉眼血尿样本当中,当红细胞的浓度达到20×10^(9)/L时,白细胞酯酶的检测项目开始出现弱阳性(±)的结果,并从红细胞的浓度达到50×10^(9)/L至200×10^(9)/L,白细胞酯酶的检测项目表现为阳性(+)结果。结论肉眼血尿当中红细胞能够对白细胞酯酶检测结果准确性造成影响,导致其出现假阳性,若想减少此不良干扰,可通过离心取上层清液后处理再进行尿干化学分析仪检测,以达到降低检测结果假阳性率的目的。
Objective To explore and analyze the effect of red blood cells in gross hematuria samples on leukocyte esterase and the treatment measures.Methods 100 gross hematuria specimens were collected in our hospital from January 2020 to January 2021,and a urine dry chemistry analyzer was used to detect the level of leukocyte esterase in the urine specimens,and then centrifuge the hematuria specimens with a centrifuge,and the levels of leukocyte esterase were detected again using a urine dry chemical analyzer.Traditional manual sediment microscopy was used as the gold standard to compare the false positive rate of urine dry chemical analyze method before and after centrifugation.The relationship between the concentration of different red blood cells and the positive and negative results of white blood cell esterase was analyzed.Results The false positive rate of leukocyte esterase in gross hematuria specimens before centrifugation was 73/(73+4)×100%=94.81%.The false positive rate of leukocyte esterase after centrifugation of gross hematuria specimens was 25/(25+52)×100%=32.47%.The false positive rate of leukocyte esterase in gross hematuria specimens after centrifugation was lower than the false positive rate of leukocyte esterase in gross hematuria specimens before centrifugation,and the difference was statistically significant(P<0.05).When analyzing the relationship between different red blood cell concentrations and the positive and negative white blood cell esterase test,it can be seen that in gross hematuria samples,when the red blood cell concentration reaches 20×109/L,the leukocyte esterase test item begins to show a weakly positive(±)result.And the concentration of red blood cells reaching 50×109/L to 200×109/L,the leukocyte esterase test items showed a positive(+)result.Conclusions The red blood cells in the gross hematuria could affect the accuracy of the leukocyte esterase test results,leading to false positives.Process the urine dry chemical analyze by take the supernatant after centrifugation,could achi
