摘要
目的探讨复健通络汤对开颅减压术治疗外伤性脑出血术后患者生活能力、神经功能及血液学指标的影响。方法选取2013年5月—2020年6月收住入院的104例外伤性颅内出血患者为研究对象,按住院号单双平均分成2组。均予标准大骨瓣减压术,对照组(52例)予控制感染、降低颅内压、减轻脑水肿等对症治疗,研究组(52例)在对照组基础上术后加用复健通络汤。治疗3周,观察2组术后第1天、术后2周在生活能力评分、神经功能、炎症因子、氧化应激指标、昏迷量表等变化并比较。结果2组术后第1天各指标比较差异无统计学意义(P>0.05),2组术后2周神经生长因子(NGF)、脑源性神经营养因子(BDNF)较术后第1天均显著升高(P<0.05),且研究组显著高于对照组(P<0.05)。2组术后第1天各指标比较差异无统计学意义(P>0.05),2组术后2周TNF-α、IL-1、IL-6、IL-8较术后第1天均显著降低(P<0.05),且研究组优于对照组(P<0.05)。2组术后第1天各指标比较差异无统计学意义(P>0.05),2组术后2周丙二醛(MDA)较术后第1天均显著下降,血清超氧化物歧化酶(SOD)、还原谷胱甘肽(GSH)、一氧化氮(NO)、血浆内皮素(ET)较术后第1天均显著上升(P<0.05),且研究组显著优于对照组(P<0.05)。2组术后第1天各指标比较差异无统计学意义(P>0.05),2组术后2周简式Fugl-Meyer评定量表(FMA)、格拉斯昏迷评分量表(GCS)、Barthel指数(BI)较术后第1天显著升高,美国国立卫生院神经功能缺损评分(NIHSS)较术后第1天均显著降低(P<0.05),且研究组优于对照组(P<0.05)。结论复健通络汤能改善开颅减压术后外伤性颅内出血患者神经功能,抑制炎症反应,抑制氧化应激反应,促进神经功能,提高生活能力。
Objective To investigate the effect of Fujian Tongluo decoction on life ability,neurological function and hematological indexes of patients with traumatic cerebral hemorrhage after craniotomy decompression.Methods 104 patients with traumatic intracranial hemorrhage admitted to our hospital from August 2013 to May 2020 were selected as the research objects.According to the hospitalization number,they were divided into two groups,all of which were given standard large bone flap decompression.The control group(n=52)was given symptomatic treatment such as infection control,intracranial pressure reduction and brain edema reduction.The study group(n=52)was given Fujian Tongluo decoction on the basis of the control group for 3 weeks The scores of life ability,neurological function,inflammatory factors,oxidative stress and coma scale were compared on the first day and the second week after operation.Results There was no significant difference between the two groups on the first day after operation(P>0.05).The levels of nerve growth factor(NGF)and brainderived neurotrophic factor(BDNF)in the two groups at 2 weeks after operation were significantly higher than those on the first day after operation(P<0.05),and the levels in the study group were significantly higher than those in the control group(P<0.05).There was no significant difference between the two groups on the first day after operation(P>0.05).The levels of tumor necrosis factor-α(TNF-α),interleukin-1(IL-1),IL-6 and IL-8 in the two groups were significantly lower than those on the first day after operation(P<0.05),and the study group was better than the control group(P<0.05).There was no significant difference between the two groups on the first day after operation(P>0.05).The malondialdehyde(MDA)in the two groups at 2 weeks after operation was significantly lower than that on the first day after operation,and the serum superoxide dismutase(SOD),reduced glutathione(GSH),nitric oxide(no)and plasma endothelin(ET)in the study group were significantly higher than th
