摘要
目的探讨低频双侧交替经颅刺激联合强制性使用运动疗法(constraint-induced movement therapy,CIMT)治疗痉挛型脑瘫患儿对其核心控制能力、精细功能及髓鞘碱性蛋白(myelin basic protein,MBP)水平的影响。方法纳入中山市人民医院2018年2月至2021年2月收治的120例痉挛型脑瘫患儿作为研究对象,通过随机数字表法分为研究组和对照组,各60例。对照组男31例,女29例,年龄(3.87±2.01)岁,给予低频双侧交替经颅刺激治疗;研究组男32例,女28例,年龄(3.76±1.88)岁,在对照组基础上联合CIMT治疗。比较两组治疗前后核心控制能力、精细功能及MBP水平。结果治疗后,两组粗大运动评分明显提升,且研究组显著高于对照组[(55.67±2.28)分比(49.43±2.29)分,t=14.957,P<0.05]。研究组中立坐位核心肌群表面肌电图值(37.43±6.47)、(31.58±7.12)、(27.63±5.35)、(27.66±6.03),明显优于对照组的(29.03±5.66)、(24.87±5.23)、(22.53±3.27)、(24.36±3.17)(t=7.549、5.883、6.300、3.752,均P<0.05)。研究组训练后精细运动功能测试量表评分(70.33±9.02)分、(15.15±0.23)分、(26.41±1.18),优于对照组的(63.17±13.48)分、(14.75±0.77)分、(24.57±4.33)分(t=3.419、3.856、3.176,均P<0.05)。治疗后,两组患者MBP水平较治疗前均有所下降(t=2.624、7.993,均P<0.05),且研究组较对照组更低[(1.56±0.63)μg/L比(2.01±0.83)μg/L,t=3.345,P<0.05]。结论低频双侧交替经颅刺激联合CIMT治疗痉挛型脑瘫患儿可显著提升其核心控制能力和精细功能,降低MBP水平,提高临床疗效,值得推广。
Objective To explore the influence of low-frequency bilateral alternating transcranial stimulation combined with constraint-induced movement therapy(CIMT)on the core control ability,fine function,and myelin basic protein(MBP)level in children with spasmodic cerebral palsy.Methods A total of 120 children with spasmodic cerebral palsy admitted to Zhongshan People's Hospital from February 2018 to February 2021 were included as the research objects,and were divided into a research group and a control group by the random number table method,with 60 cases in each group.There were 31 boys and 29 girls in the control group,with an age of(3.87±2.01).There were 32 boys and 28 girls in the research group,with an age of(3.76±1.88).The control group were treated with low-frequency bilateral alternating transcranial stimulation;in addition,the research group were treated with CIMT.The core control abilities,fine function and MBP levels in the two groups before and after the treatment were compared.Results After the treatment,the gross motor scores of the two groups were significantly increased,and the gross motor score in the research group was significantly higher than that in the control group[(55.67±2.28)vs.(49.43±2.29),t=14.957,P<0.05)].The values of surface electromyography of the core muscle group in the neutral sitting position were(37.43±6.47),(31.58±7.12),(27.63±5.35),and(27.66±6.03)in the research group,and were(29.03±5.66),(24.87±5.23),(22.53±3.27),and(24.36±3.17)in the control group(t=7.549,5.883,6.300,and 3.752;all P<0.05).After the treatment,the scores of Fine Motor Function Measure were(70.33±9.02),(15.15±0.23),and(26.41±1.18)in the research group,and were(63.17±13.48),(14.75±0.77),and(24.57±4.33)in the control group(t=3.419,3.856,and 3.176,all P<0.05).After the treatment,the MBP levels of the two groups decreased(t=2.624 and 7.993;both P<0.05);and the MBP level in the research group was lower than that in the control group[(1.56±0.63)μg/L vs.(2.01±0.83)μg/L,t=3.345,P<0.05].Conclusion Low-f
