摘要
目的比较胃癌根治术后淋巴结精细分拣与区块分拣的临床应用价值。方法选取116例行标准胃癌根治术的患者为研究对象,根据术后按离体标本淋巴结取材方式,将患者随机分为A组(59例)、B组(57例)。A组采用精细分拣法摘取离体标本淋巴结;B组采用区块分拣法摘取离体标本淋巴结;记录每组患者淋巴结取材所用时间、术后病理证实的淋巴结清扫总数和转移性淋巴结数目。结果A组淋巴结清扫总数多于B组淋巴结总数,差异有统计学意义(P<0.05);A组与B组淋巴结阳性数目比较,差异无统计学意义(P>0.05);A组与B组N分期比较,差异无统计学意义(P>0.05);A组取材耗时多于B组,差异有统计学意义(P<0.05)。结论胃癌根治术后标本采用区块分拣方法方便快捷,检出淋巴结总数达到评估病情标准,适合当前临床推广应用。
Objective To compare the clinical value of fine lymph node sorting and regional lymph node sorting from gastric cancer samples after curative gastrectomy.Methods Totally 116 patients who underwent standard curative gastrectomy were retrospectively included and analyzed.On the basis of the methods of sorting lymph nodes from the gastric samples after gastrectomy,the patients were divided into either group A(59 cases)or group B(57 cases).Lymph nodes were extracted by fine sorting method in group A,while by regional sorting method in group B.The duration time of lymph node sorting in each group was recorded.Both the numbers of examined lymph nodes and metastatic lymph nodes were collected and compared between the two groups.Results The total number of lymph nodes in group A was higher than that in group B,and the difference was statistically significant(P<0.05).The number of metastatic lymph nodes between two groups showed no statistical significance(P>0.05).There was no statistical significance in the N stage between two groups(P>0.05).The duration time of sorting in group A was longer than that in group B,and the difference was statistically significant(P<0.05).Conclusion Regional lymph node sorting after curative gastrectomy is convenient and time-saving.Meanwhile the total number of lymph nodes meets the criteria for disease evaluation,which is suitable for clinical application.
