^(18)F-FDG PET/CT在预测非小细胞肺癌远处转移的价值研究 被引量:10

The Value of ^(18)F-FDG PET/CT in Predicting Distant Metastasis of Non-Small Cell Lung Cancer

摘要 目的探讨氟代脱氧葡萄糖(^(18)F-FDG)正电子发射断层成像/X线计算机体层成像(PET/CT)标准化摄取值(SUV)、肿瘤代谢体积(MTV)和病灶糖酵解总量(TLG)在预测非小细胞肺癌(NSCLC)远处转移中的价值。方法回顾性分析2015年1月至2020年12月在广东医科大学附属医院行^(18)F-FDG PET/CT检查并经手术病理确诊为NSCLC的171例患者相关资料。分析临床病理资料和肿瘤原发灶各代谢参数与远处转移的相关性。利用二元Logistic回归分析,确定预测远处转移的独立危险因子。结果171例入组患者中,NSCLC远处转移组最大标准摄取值(SUV_(max))、平均标准摄取值(SUV_(mean))、MTV、TLG均较无远处转移组高(P值分别为0.001、0.001、<0.001、<0.001)。Logistic回归多因素分析示肿瘤最大径、MTV是预测远处转移的独立因素(P<0.05)。结论肿瘤最大径、MTV是预测NSCLC远处转移的独立危险因素,在预测NSCLC远处转移中分别具有一定参考价值。 Objective To investigate the value of ^(18)F-fluorodeoxyglucose(^(18)F-FDG)PET/CT standardized uptake value(SUV),tumor metabolic volume(MTV)and total glycolysis(TLG)in predicting distant metastasis of(NSCLC)in non-small cell lung cancer(NSCLC).Methods The data of 171 patients who underwent ^(18)F-FDG PET/CT examination and were diagnosed as NSCLC by surgery and pathology in the affiliated hospital of Guangdong medical university from January 2015 to December 2020 were retrospectively analyzed.The correlation between the clinicopathological data and the metabolic parameters of the primary tumor and distant metastasis was analyzed.Binary Logistic regression analysis was used to determine the independent risk factors for predicting distant metastasis.Results SUV_(max),SUV_(mean),MTV and TLG in NSCLC distant metastasis group were higher than those in non-distant metastasis group(P=0.001,0.001,<0.001,<0.001,respectively).Logistic regression multivariate analysis showed that the maximum diameter of tumor and MTV were independent factors for predicting distant metastasis.Conclusion the maximum diameter of tumor and MTV are independent risk factors for predicting distant metastasis of NSCLC,and they have certain reference value in predicting distant metastasis of NSCLC.
