摘要
目的:应用二维斑点追踪成像(2D-STE)技术评价不同血糖状态对高血压患者左心房功能的影响。方法:根据血糖水平将2017年7月~2020年6月住院治疗且被诊断为高血压的患者分为糖尿病组(n=42)、糖尿病前期组(n=38)和血糖正常组(n=56),应用二维超声技术和2D-STE对三组患者进行检查,比较三组患者的检查结果。结果:与糖尿病前期组和血糖正常组相比,糖尿病组:A值(心房收缩期最大血流速度峰值)显著增大(P<0.05)、e'值(舒张早期室间隔侧二尖瓣环根部运动速度峰值)显著减少(P<0.05)、E(二尖瓣口舒张早期血流速度峰值)/e'比值显著增大(P<0.05)、EDS值(左心房舒张早期纵向应变值)和SRE值(左房舒张早期纵向应变率)显著减少(P<0.05)、LDS值(舒张末期纵向应变值)显著增大(P<0.05),以上差异均有统计学意义(P<0.05)。结论:糖尿病可使高血压患者左心房的“管道”和“增压泵”功能受损。
Objective To explore the effect of glycemic state levels on the left atrial function in hypertensive patients by two-dimensional speckle tracking echocardiography(2D-STE).Method The patients with hypertension were selected from July 2017 to June 2020 in our hospital were divided into the diabetes group(n=42),prediabetes group(n=38)and normal blood glucose group(n=56)according to blood glucose level.The two-dimensional ultrasound technology and 2D-STE were applied to examine the patients among three groups and the examination results were compared.Results In comparison with prediabetes group and normal blood glucose group,the peak value of atrial systolic maximum blood flow velocity(A)was significantly increased(P<0.05),the peak velocity of mitral annulus root(e')was significantly decreased in early diastolic phase(P<0.05),Peak value of early diastolic velocity of mitral valve orifice(E)/e'was significantly increased(P<0.05);The left atrial early diastolic longitudinal strain(EDS)and left atrial early diastolic longitudinal strain rate(SRE)decreased significantly(P<0.05),while the left atrial late diastolic longitudinal strain(LDS)was significantly increased(P<0.05).Conclusion Diabetes could damage the conduit function and contraction function of left atrium in patients with hypertension.
