摘要
目的探究改良经尿道前列腺等离子剜除术(PKRP)治疗良性前列腺增生(BPH)的疗效及对性功能的影响。方法选取2018年1月至2021年4月眉山市彭山区人民医院诊治的100例BPH患者的临床资料进行回顾性分析。其中行改良PKRP治疗的54例,设为改良组;行常规PKRP治疗的46例,设为PKRP组。采用国际前列腺症状评分法(IPSS)和生活质量评分表(QOL)评估两种手术方法的临床疗效;采用国际勃起功能指数(IIEF-5)、勃起障碍率和逆行射精率评价两组术后性功能。结果改良组手术时长、手术出血量、术后导尿管留置时间和术后住院时间均少于PKRP组,差异具有统计学意义(P<0.05);改良组术后并发症发生率为22.22%,PKRP组术后并发症发生率为32.61%,差异无统计学意义(P>0.05);术后3个月,改良组最大尿流率和IIEF-5评分明显高于PKRP组,IPSS评分、QOL评分、勃起功能障碍率和逆行射精率均明显低于PKRP组,差异具有统计学意义(P<0.05)。结论改良PKRP在最大限度切除增生组织前提下,能取得良好的临床效果,维持患者性功能水平。
Objective To explore the curative effect of modified transurethral plasma resection of the pros-tate(PKRP)on benign prostatic hyperplasia(BPH)and its influences on sexual function.Methods Clinical data of 100 BPH patients diagnosed and treated in Pengshan District People′s Hospital of Meishan from January 2018 to April 2021 were retrospectively analyzed.Among them,54 BPH patients treated with modified PKRP were set as the modified group.46 patients receiving routine PKRP treatment for BPH were included in the PKRP group.The inter-national prostate symptom score(IPSS)and quality of life scale(QOL)were used to evaluate the clinical efficacy of the two surgical methods in the treatment of BPH.International index of erectile function(IIEF-5),erectile dys-function rate and retrograde ejaculation rate were used to evaluate the postoperative sexual function of the two groups.Results The operative duration,operative blood loss,postoperative catheter indwell time and postoperative hospital stay in the modified group were all shorter than those in the PKRP group(P<0.05).The incidence of postoperative complications was 22.22%in the modified group and 32.61%in the PKRP group,and the difference was statistically significant(P>0.05).Three months after surgery,the maximum urine flow rate and IIEF-5 score in the modified group were significantly higher than those in the PKRP group,and the IPSS score,QOL score,erec-tile dysfunction rate and retrograde ejaculation rate were significantly lower than those in the PKRP group(P<0.05).Conclusions Modified PKRP can achieve good clinical effect and maintain patients′sexual function on the premise of maximum excision of hyperplastic tissue.
