摘要
目的探究隐匿性阴茎患儿采用包皮脱套松解延长固定术(改良Devine术)治疗对阴茎疲软时显露长度的影响。方法选取2018年2月至2021年1月蚌埠市第一人民医院诊治的70例隐匿性阴茎患儿的临床资料进行回顾性分析。给予改良Devine术,统计患儿手术时间、术中出血量、术后并发症、阴茎回缩率及水肿消退时间,对比手术前后阴茎体显露长度。结果手术患儿都顺利开展手术,不存在尿道损伤、神经损伤及血管损伤,均快速康复,术后第1天可出院。术前阴茎体显露长度(1.42±0.36)cm,治疗1周后(4.32±0.74)cm,治疗4周后(4.69±0.78)cm,治疗12周后(4.95±0.75)cm,治疗1周后、治疗4周后、治疗12周后阴茎体显露长度均长于术前,差异具有统计学意义(P<0.05)。本组术后1例疤痕赘生,无狭窄环、感染、水肿,并发症发生率为1.43%(1/70),无术后阴茎回缩,外形满意度为97.14%(68/70)。全部患儿均随访半年,全部患儿阴茎都能够伸直,包皮能够翻起且将阴茎头部充分显露,均无尿管损伤、神经损伤及血管损伤。结论改良Devine术能够提高阴茎疲软时显露长度,术后并发症发生率低,外形满意度高。
Objective To explore the influence of prepuce release and prolonged fixation(modified De-vine)on the exposed length of the penis in children with occult penile weakness.Methods The clinical data of 70 pediatric patients with concealed penis admitted to Bengbu First People′s Hospital from February 2018 to January 2021 were reviewed and given modified Devine surgery.The operation time,intraoperative blood loss,postoperative complications,penile shrinkage rate and time of edema regression were counted,and the penis exposure length at different times was compared.Results The operation was carried out smoothly in all the children without urethral injury,nerve injury or blood vessel injury.The exposure length of the penis was(1.42±0.36)cm before treat-ment,(4.32±0.74)cm after 1 week of treatment,(4.69±0.78)cm after 4 weeks of treatment,and(4.95±0.75)cm after 12 weeks of treatment.In this group,compared with preoperative,there were significant differences in the exposure length of the penis after 1 week of treatment,4 weeks of treatment,and 12 weeks of treatment(P<0.05).In this group,there was 1 case of postoperative scar hypertrophy,no stenosis,infection,edema,complica-tion rate was 1.43%(1/70),no postoperative penile retraction,and shape satisfaction was 97.14%(68/70).All the children were followed up for half a year.All the children had the penis unbent and foreskin turned up,and the head of the penis was fully exposed,without urinary tube injury,nerve injury or blood vessel injury.Conclusions The operation can improve the exposure length when the penis is weak,the incidence of postoperative complications is low,and the appearance satisfaction is high.
