摘要
目的探讨血清细胞角蛋白19片段(CYFRA21-1)、血管内皮生长因子(VEGF)、甲胎蛋白(AFP)、糖类抗原19-9(CA19-9)和鳞状细胞癌抗原(SCC)对食管癌的诊断价值。方法选取2016年2月至2021年2月间西安市长安医院收治的92例食管癌患者为食管癌组,84例食管良性疾病患者为食管良性疾病组,同期88例健康体检者为对照组。比较三组受试者血清CYFRA21-1、VEGF、AFP、CA19-9和SCC水平以及不同TNM分期、有无淋巴结转移的食管癌患者以上指标水平。采用受试者工作特征曲线(ROC曲线)评估五项指标及其联合检测对食管癌的诊断价值。结果食管癌组血清CY-FRA21-1、VEGF、AFP、CA19-9和SCC水平均高于食管良性疾病组和对照组,差异均有统计学意义(均P<0.05),而食管良性疾病组与对照组的上述水平比较,差异无统计学意义(P>0.05)。Ⅲ~Ⅳ期的食管癌患者五项血清指标水平均高于Ⅰ~Ⅱ期患者,有淋巴结转移的食管癌患者五项血清指标水平高于无淋巴结转移患者,差异均有统计学意义(均P<0.05)。ROC曲线显示,诊断食管癌的曲线下面积(AUC)、敏感度和特异度分别是,CYFRA21-1为0.749、77.2%、71.4%,VEGF为0.852、82.6%、85.7%,AFP为0.631、63.0%、67.9%,CA19-9为0.819、83.7%、81.0%,SCC为0.717、72.8%、72.6%;五项联合诊断的AUC为0.960,敏感度90.2%,特异度91.7%,高于各指标单项检测。结论血清CYFRA21-1、VEGF、AFP、CA19-9和SCC联合检测可有效诊断食管癌并评估病情,为临床诊治提供依据。
Objective To investigate the significance of detection of serum cytokeratin 19 fragment( CYFRA21-1),vascular endothelial growth factor( VEGF),alpha-fetoprotein( AFP),carbohydrate antigen 19-9( CA19-9) and squamous cell carcinoma antigen( SCC) for the diagnosis of esophageal cancer.Methods From February 2016 to February 2021,92 patients with esophageal cancer admitted to Chang’an Hospital were selected and included in an esophageal cancer group. In addition,84 patients with benign esophageal disease were included in benign esophageal disease a group and 88 healthy individuals who had physical examination in the same period were included in a control group. Serum CYFRA21-1,VEGF,AFP,CA19-9 and SCC levels were compared among the three groups and the levels of the above indicators were also compared in esophageal cancer patients of different tumor node metastasis( TNM) staging and patients with or without lymph node metastasis. The receiver operating characteristic curve( ROC) curve was used to evaluate the diagnostic value of the five indicators and their combination detection for esophageal cancer. Results Serum levels of CYFRA21-1,VEGF,AFP,CA19-9,and SCC were higher in the esophageal cancer group than those in the benign esophageal disease group and the control group( all P < 0. 05).However,there was no statistical significance in the above-mentioned levels between the esophageal benign disease group and the control group( P > 0. 05). The serum levels of the five serum indicators were higher in patients with stage Ⅲ to Ⅳ esophageal cancer than those in patients with stage Ⅰ to Ⅱ esophageal cancer( all P < 0. 05). The serum levels of the above five biomarkers were higher in patients with lymph node metastasis than in those without lymph node metastasis( all P < 0. 05). The ROC curve showed that the area under the curve( AUC),sensitivity,and specificity for diagnosis of esophageal cancer were 0. 749,77. 2%and 71. 4%,respectively for CYFRA21-1,0. 852,82. 6% and 85. 7%,respectively for VEGF,0. 631,63. 0% and
