摘要
目的:初步探讨仰卧平髋位进行婴儿髋关节超声检查的临床可行性。方法:随机选取髋关节常规超声检查的就诊婴儿,检查体位包括侧卧屈曲位和仰卧平髋位,成像标准与测量方法均选用Graf法,测量指标包括α角和β角。采用配对设计资料检验方法比较同一医生2种体位和同一体位(侧卧屈曲位或仰卧平髋位)不同医生的测量结果。结果:所有婴儿均成功完成侧卧屈曲位和/或仰卧平髋位的超声检查,同一医师2种体位的α角和β角测值差异无统计学意义(P>0.05),不同医师采用仰卧平髋位检查同一婴儿的测值(α角和β角)差异无统计学意义(P>0.05),不同医师采用侧卧屈曲位检查同一婴儿时的α角测值差异无统计学意义(P>0.05),不同医师采用侧卧屈曲位检查同一婴儿的β角测值差异有统计学意义(P<0.05)。结论:进行婴儿髋关节超声检查时,可以采用仰卧平髋位,与侧卧屈曲位比较,该体位易于确保每次检查的婴儿体位固定统一,值得开展进一步的临床应用研究。
Purpose:To study the clinical feasibility of ultrasonic examination of infant hip joint with supine hip position(SHP).Methods:The infants who undergone routine ultrasound examination were randomly selected.The body positions of infants included the SHP and the lateral flexion position(LFP).The Graf method was used for imaging and measuring.The measurement indicators included the angle ofαandβ.Measurements from the same doctor with two kinds of body positions and different doctors with the same body position were compared.The paired design data test was used to compare statistical difference of the measurement result.Results:All ultrasound examination of infants were completed successfully with body position of SHP and/or LFP.There was no significant difference ofαandβbetween two body positions for the same sonographer(P>0.05).There was no significant difference ofαandβbetween two sonographers with the body position of SHP for the same infant(P>0.05).There was no significant difference ofαbetween two sonographers with the body position of SHP for the same infant(P>0.05).There was significant difference ofβbetween two sonographers with the body position of LFP for the same infant(P<0.05).Conclusions:When the same sonographer performs the ultrasonic examination of infant hip joint,the SHP may be choosen for the body position of infant.Compared with LFP,SHP is more likely to ensure that the baby position is fixed and unified for each examination.It is worthy of further clinical application research.
