OCTA应用于糖尿病视网膜病变的研究进展 被引量:11

An Update on Application of Optical Coherence Tomography Angiography in Diabetic Retinopathy

摘要 糖尿病视网膜病变(DR)是糖尿病的一种常见并发症,其破坏视网膜微血管,是全球眼病患者视力下降的主要原因。目前,检测DR的金标准是眼底荧光素血管造影(FFA)技术。随着光学相干断层血管造影成像(OCTA)的出现,其提供了一种快速、安全的方法来评估视网膜微血管系统,比传统的荧光血管造影具有更高的对比度和分辨率。高质量的OCTA图像使视网膜和脉络膜毛细血管、微动脉瘤和新生血管变得可视化,它的自动分析可量化视网膜毛细血管和脉络膜毛细血管的改变。因此,OCTA作为一种客观的DR检测技术有着广阔的应用前景,然而,OCTA并非全能。笔者将对不同阶段DR在OCTA下的表现以及OCTA存在的局限性进行综述。 Diabetic retinopathy(DR),a common diabetic complication,predominantly affecting the capillaries in the retina,is a leading cause of blindness worldwide.Currently,fluorescein angiography(FFA)has been the gold standard for vascular imaging of DR.Recent advances in optical coherence tomography angiography(OCTA)enable a rapidly evolving technique that provides a fast,safe assessment of retinal microvascular flow.OCTA visualizes the retinal microvasculature with higher contrast and better resolution than FFA.OCTA,producing high-quality images,is a novel imaging tool which allows the visualization of features associated with DR,including microaneurysms and neovascularization and the quantification of alterations in retinal capillary and choriocapillaris.Therefore,OCTA as a quantitative and objective technique has promising application in the field of DR classification.Nevertheless,OCTA has several limitations that need to be addressed.This paper reviewed manifestations and limitations of OCTA in different stages of DR.
