摘要
目的探讨低条件斜冠状位容积扫描模式CT在骶髂关节检查中实现低辐射剂量的可行性。资料与方法前瞻性收集104例需行骶髂关节CT检查的患者,根据随机数表法分为A组(轴位螺旋扫描组)、B组(斜冠螺旋扫描组)、C组(轴位volume扫描组)、D组(斜冠状位volume扫描组),每组26例,行不同低剂量扫描方案。比较各组图像的有效剂量和图像质量评分。评估2名观察者对各组骶髂关节的诊断一致性。结果①A、B、C、D组有效剂量分别为1.28(1.18,1.34)mSv、0.94(0.89,0.94)mSv、0.59(0.59,0.59)mSv、0.46(0.39,0.51)mSv,差异有统计学意义(H=92.348,P<0.01)。②2名观察者对4组图像的主、客观图像质量评分比较,差异无统计学意义(主观质量评分:H=0.03,P>0.05;H=0.41,P>0.05;客观质量评分:H=1.13,P>0.05;H=2.32,P>0.05),2名观察者主、客观评分具有高度一致性(加权Kappa值>0.810;客观评分ICC>0.810)。③2名观察者诊断分级结果的加权Kappa值>0.810,2名观察者对4组图像的诊断一致性好。结论在保证图像质量的前提下,低条件斜冠状位容积扫描模式骶髂关节CT可以显著降低辐射剂量。
Purpose To explore the feasibility of low radiation dose in sacroiliac joint examination used by the low-condition semicoronal volume CT scanning.Materials and Methods A total of 104 patients requiring CT examination of the sacroiliac joint were prospectively collected,and divided into four groups with different low-dose scanning regiments according to random number table.There were 26 cases in each group,including group A(axial spiral scanning group),group B(semicoronal spiral scanning group),group C(axis volume scanning group)and group D(semicoronal volume scanning group).The effective dose and image quality score of each group were compared,and the diagnostic consistency of the sacroiliac joints in each group was assessed by two observers,respectively.Results 1.The effective doses of groups A,B,C and D were 1.28(1.18,1.34)mSv,0.94(0.89,0.94)mSv,0.59(0.59,0.59)mSv,0.46(0.39,0.51)mSv,respectively.There was no statistically significant difference in effective dose among four groups(H=92.348,P<0.01).2.There were no significant differences in subjective and objective image quality scores between the two observers(Subjective score:H=0.03,P>0.05;H=0.41,P>0.05;Objective score:H=1.13,P>0.05;H=2.32,P>0.05).The subjective and objective scores of the four groups of images were highly consistent(weighted Kappa>0.810;objective score ICC>0.810).3.The weighted Kappa value of the two observers'diagnostic grades was>0.810,two observers showed good diagnostic consistency among four groups.Conclusion The low condition semicoronal volume scanning can significantly reduce the radiation dose of sacroiliac joint CT on the premise of ensuring image quality.
