阳江地区产前血清学筛查高风险孕妇的羊水细胞染色体核型分析的研究 被引量:2

摘要 目的探究阳江地区产前血清学筛查高风险孕妇的羊水细胞染色体核型分析的结果。方法2018年4月至2021年1月阳江市妇幼保健院收治的血清学阳性的730例高风险孕妇做染色体核型检测,知情选择情况下行羊膜腔穿刺术与染色体核型检测。记录羊水细胞培养成功率情况、培养时间,分析异常染色体核型结果(数目与结构异常、高危指征染色体异常)。结果羊水细胞一次性培养成功724例(99.18%),两次培养成功729例(99.86%);羊水细胞最短培养时间为6 d,最长为15 d。729例染色体核型中,发现异常核型98例(13.44%),其中数目异常50例(6.86%),结构异常25例(3.43%),倒位19例(2.61%),嵌合体4例(0.55%)。产前诊断血清学异常染色体检出率从高到低分别为夫(妇)染色体异常、超声异常、高龄妊娠、不良孕产史与唐氏筛查高风险。结论阳江地区产前血清学筛查高风险孕妇的羊水细胞染色体核型异常率较高,羊水细胞染色体核型分析可准确发现胎儿染色体异常,为临床治疗及终止妊娠提供科学依据。 Objective To investigate the results of karyotype analysis of amniotic fluid cells in high-risk pregnant women with prenatal serological screening in Yangjiang area.Methods A total of 730 high-risk pregnant women due to positive serology were admitted to Yangjiang Maternal and Child Health Hospital for karyotyping from April 2018 to January 2021,and amniocentesis and karyotype testing was performed under informed choice.The success rate of amniotic fluid cell culture,culture time and analysis of abnormal karyotype results(abnormalities in number and structure,chromosome abnormalities in high-risk indications)were recorded.Results 724 cases(99.18%)of amniotic fluid cells were successfully cultured at one time and 729 cases(99.86%)were cultured twice.The shortest culture time for amniotic fluid cells was 6 d and the longest was 15 d.There were 98 cases(13.44%)of abnormal karyotypes among the 729 karyotypes,including 50 cases(6.86%)of numerical abnormalities,25 cases(3.43%)of structural abnormalities,and 19 cases(2.61%)of inversions and 4 cases(0.55%)of chimerism.The detection rate of chromosome abnormalities in prenatal serology from highest to lowest were chromosome abnormalities in husband(woman),ultrasound abnormalities,advanced pregnancy,adverse pregnancy history and high risk of Down's syndrome screening,respectively.Conclusion The abnormal rate of karyotype of amniotic fluid cells was higher among highrisk pregnant women with prenatal serological screening in Yangjiang area.The karyotype analysis of amniotic fluid cells can accurately detect fetal chromosome abnormalities and provide the scientific basis for clinical treatment and termination of pregnancy.
