摘要
目的探讨自体富血小板纤维蛋白(platelet rich fibrin,PRF)联合负压封闭引流(vacuum sealing drainage,VSD)技术治疗难治性哺乳期乳腺炎溃疡创面的临床疗效。方法自2016年2月至2019年10月,回顾性分析北部战区总医院烧伤整形科14例哺乳期女性患乳腺炎后出现较大溃疡创面的临床资料,采用PRF与VSD联合技术闭合创面周围腔隙的同时应用PRF+自体中厚皮片大张移植修复创面,观察并记录愈合效果。术后随访6~36个月,统计患者治疗有效率、愈合时间、不良反应、创面分泌物评分、创面肉芽情况评分、VAS疼痛评分和乳房外观评分。结果本组14例患者皮片移植修复后创面均愈合,治疗时间为25~60 d,平均(34.52±3.18)d。乳房外形评判优秀8例,良好5例,一般1例。局部均无破溃、无明显瘢痕增生。结论PRF能明显促进难治性乳腺炎溃疡创面及其周围腔隙愈合。
Objective To explore the clinical application and efficacy of autologous platelet rich fibrin(PRF) combined with vacuum sealing drainage(VSD) in the treatment of large ulcer wounds in refractory lactation mastitis. Methods From February 2016 to October 2019, the clinical data of 14 patients with large ulcer wounds caused by lactation mastitis were retrospectively analyzed. The VSD and autologous PRF were used to fill cavities around the wounds to make the wounds closed. Then PRF and autologous medium-thickness skin graft were used to repair the wounds. The efficacy was observed and recorded. All patients were followed up for 6 to 36 months. The effective rate, healing time, adverse reactions, wound secretion scores, granulation growth scores, VAS pain scores and breast shape scores were analyzed and patients’ satisfaction were recorded. Results After PRF combined with VSD treatment, all wounds in 14 patients were healed after autologous medium-thickness skin graft repair. The treatment time was 25 to 60 days, with an average of(34.52±3.18) days.The evaluation of breast shape was excellent in 8 cases, good in 5 cases and general in 1 case. During the follow-up, no local ulceration or obvious scar hyperplasia occurred. Conclusion PRF can significantly promote the healing of ulcer wounds and surrounding cavities in patients with refractory lactation mastitis.
