摘要
目的分析来氟米特联合糖皮质激素治疗狼疮性肾炎的效果与安全性。方法选取2017年3月至2020年3月本院收治的102例狼疮性肾炎患者作为研究对象,将其随机分为对照组和联合组,每组51例。对照组给予糖皮质激素治疗,联合组在对照组基础上给予来氟米特治疗。比较两组的临床疗效、不良反应发生情况、炎症及肾小球滤过功能指标、疾病活动度评分。结果联合组的治疗总有效率显著高于对照组,差异具有统计学意义(P<0.05)。两组的不良反应总发生率无显著差异(P>0.05)。治疗前,两组的CRP、IL-17、BUN水平及eGFR无显著差异(P>0.05);治疗后,两组的CRP、IL-17、BUN水平均降低,eGFR均升高,且联合组显著优于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的SLEDAI评分无显著差异(P>0.05);治疗后,两组的SLEDAI评分均降低,且联合组低于对照组,差异具有统计学意义(P<0.05)。结论来氟米特联合糖皮质激素治疗狼疮性肾炎的效果较好,安全性较高,可减轻炎症反应,改善肾小球滤过功能和疾病活动度。
Objective To analyze the efficacy and safety of leflunomide combined with glucocorticoid in the treatment of lupus nephritis.Methods A total of 102 patients with lupus nephritis treated in our hospital from March 2017 to March 2020 were selected as the research objects and randomly divided into control group and combined group,with 51 cases in each group.The control group was treated with glucocorticoid,and the combined group was treated with leflunomide on the basis of the control group.The clinical efficacy,adverse reactions,the indexes of inflammation and glomerular filtration function,and disease activity score were compared between the two groups.Results The total effective rate of treatment in the combined group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Before treatment,there were no significant differences in CRP,IL-17,BUN levels and eGFR between the two groups(P>0.05);after treatment,CRP,IL-17 and BUN levels in the two groups decreased,eGFR increased,and those in the combined group were significantly better than the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in SLEDAI score between the two groups(P>0.05);after treatment,the SLEDAI score of the two groups decreased,and that in the combined group was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion Leflunomide combined with glucocorticoid in the treatment of lupus nephritis is effective and safe.It can reduce inflammatory reaction and improve glomerular filtration function and disease activity.
