摘要
国家医保谈判药品"双通道"实施过程中,谈判药品在院外完成处方调配、供应、使用和监管行为,脱离了医院治疗控制系统和费用控制系统,存在较大的用药安全风险和基金控费风险。本文提出"两职能、一资质"管理模式,即完善DTP(direct to patient,即直面患者)药房"用药管理""审核管理"两项管理职能,以控制用药风险和基金风险;实施"DTP药房资质认证管理"资质遴选模式,强化DTP药房药学服务专业性,推进DTP药房成为"双通道"药品供应的主要端口,做好与患者、医生、供应商及医保经办管理方等多方主体配合协调,充分发挥DTP药房的前端驱动力。
With the implementation of the "dual channel" for the national reimbursement negotiation drugs,the negotiation drugs prescription deployment,supply,use and supervision are all completed outside the hospital,which are separated from the hospital’s treatment control system and cost control system.Thus,drug safety risks and medical insurance fund risks are high in this process.This article proposed a "two functions,one qualification" management model.That is,the two management functions of "medication management" and "audit management" of direct to paient(DTP) pharmacy managent should be improved to control medication risks and fund risks,besides,"DTP pharmacy qualification management" should be implemented to strengthen the specialty of pharmaceutical care in DTP pharmacy.Through these ways,DTP pharmacy can be promoted to become the primary port of "dual channel" drug supply,and the front driving force of DTP pharmacy can be fully played under the cooperation and coordination of patients,doctors,suppliers,and medical insurance agency.
