重症肺部感染患者血清S-ChE、BNP、NLR水平与患者预后的关系 被引量:9

Analysis of the relationship between serum S-ChE,BNP,NLR levels and prognosis in patients with severe pulmonary infection

摘要 目的探讨重症肺部感染患者血清胆碱酯酶(S-ChE)、脑钠肽(BNP)、中性粒细胞/淋巴细胞比值(NLR)水平与患者预后的关系。方法选择2017年1月-2020年1月于界首市人民医院进行治疗的重症肺部感染患者80例作为病例组,选取同期本院进行体检的健康人70名作为对照组,比较两组血清S-ChE、BNP、NLR水平变化情况;根据预后情况将重症肺部感染患者分为存活组(n=59)和死亡组(n=21),分析重症肺部感染患者预后的危险因素。结果病例组血清S-ChE水平显著低于对照组,BNP、NLR水平显著高于对照组,差异有统计学意义(P<0.05)。存活组和死亡组患者性别、年龄、合并疾病、PaO_(2)、PaCO_(2)差异无统计学意义(P>0.05);死亡组S-ChE、氧合指数显著低于存活组,BNP、NLR、急性生理与慢性健康评分(APACHEⅡ评分)、多器官功能障碍综合征评分(MODS评分)、英国胸科协会改良肺炎评分(CURB-65评分)均显著高于存活组,差异均有统计学意义(P<0.05)。多因素非条件Logistic分析显示,BNP、NLR、APACHEⅡ评分、MODS评分、CURB-65评分均是重症肺部感染患者预后的独立危险因素(P<0.05),S-ChE、氧合指数是重症肺部感染患者预后的保护因素(P<0.05)。结论血清S-ChE、BNP、NLR水平与重症肺部感染患者预后密切相关,临床上可作为预测患者预后的重要指标。 Objective To study the relationship between serum cholinesterase(S-ChE),brain natriuretic peptide(BNP),neutrophil/lymphocyte ratio(NLR)levels and prognosis in patients with severe pulmonary infection.Methods 80 patients(case group)with severe pulmonary infection who were treated in Jieshou City People’s Hospital of from January 2017 to January 2020 were selected for this study.Seventy healthy people who underwent physical examination in our hospital during the same period were selected as the control group.The changes of serum S-ChE,BNP and NLR levels between the two groups were compared.According to the prognosis,they were divided into survival group(n=59)and death group(n=21).To analyze the risk factors for prognosis of patients with severe pulmonary infection.Results The level of serum S-ChE in case group was significantly lower than that in control group,and the levels of BNP and NLR were significantly higher than those in control group(P<0.05).There were no significant differences in gender,age,comorbidities,PaO_(2)and PaCO_(2)between two groups(P>0.05).S-ChE and oxygenation index in death group were significantly lower than those in survival group,and BNP,NLR,acute physiology and chronic health evaluation(APACHEⅡ)score,multiple organ dysfunction syndrome(MODS)score and confusion,uremia,respiratory,BP,age 65 years(CURB-65)score in death group were significantly higher than those in survival group(P<0.05).Multivariate non-conditional Logistic analysis showed that BNP,NLR,APACHEⅡscore,MODS score and CURB-65 score were independent risk factors for the prognosis of severe pulmonary infection(P<0.05),and S-ChE,oxygenation index were the protective factors for the prognosis of severe pulmonary infection(P<0.05).Conclusion Serum levels of S-ChE,BNP and NLR were closely related to the prognosis of patients,and could be used as an important indicator to predict the prognosis of patients in clinical practice.
