摘要
目的探讨抗-HCV联合HCV-RNA检测在血液透析患者丙型肝炎病毒(HCV)感染早期的诊断价值探讨。方法筛选2015年1月至2016年8月期间在本院血液透析中心进行规律血液透析3个月以上的患者192例。对各血液标本分别进行抗-HCV检测、HCV-RNA定性检测、HCV-RNA定量检测。统计并比较各种检测方法对HCV的检出率。将抗-HCV联合HCV-RNA定性检测为阳性的23例患者进行HCV-RNA病毒载量测定,及血清ALT、AST水平测定,统计S/CO值。对丙肝患者HCV-RNA载量与ALT、AST、S/CO分别进行Pearson直线相关分析。结果抗-HCV检测HCV的检出率为8.47%,HCV-RNA定性及定量方法对HCV的检出率分别为10.98%、6.67%。抗-HCV检测联合HCV-RNA定性检测HCV检出率为13.61%。HCV-RNA定性检测对HCV的检出率明显高于免疫荧光定量检测方法、抗-HCV检测方法(χ^(2)=0.686,P<0.01)(χ^(2)=0.613,P<0.01)。抗-HCV联合HCV-RNA定性检测对HCV的检出率明显高于HCV-RNA定性检测方法(χ^(2)=0.893,P<0.01)。Pearsen直线相关分析结果显示,丙肝患者HCV-RNA载量与ALT、AST均呈显著正相关(r=0.9933,r=0.9732,P<0.001),与S/CO值无显著相关性(r=0.3548,P>0.05)。结论HCV-RNA定性检测较抗-HCV检测更利于维持性血液透析治疗患者HCV感染的早期诊断,两者结合能够明显提高HCV感染的检出率,同时可通过检测ALT、AST水平辅助HCV感染的诊断。
Objective To explore the early diagnosis value of anti-HCV combined with HCV-RNA detection in hemodialysis patients.Methods A total of 192 patients with regular hemodialysis at the hemodialysis center in our hospital from January 2015 to August 2016 were screened.The blood samples were tested by HCV-RNA and HCV-RNA.The detection rate of HCV was measured and compared.The HCV-RNA viral load was measured in 23 patients who were positive for HCV-RNA,and serum ALT and AST were measured,and S/CO was calculated.Pearson linear correlation analysis was performed on HCV-RNA load of HCV patients and ALT,AST and S/CO.Results The detection rate of HCV was 8.47%,HCV-RNA qualitative and quantitative methods were 10.98%and 6.67%respectively.The detection rate of HCV-RNA was 13.61%.The detection rate of HCV-RNA was significantly higher than that of immunofluorescence quantitative detection method and anti-hcv detection method(χ^(2)=0.686,P<0.01)(χ^(2)=0.613,P<0.01).The detection rate of HCV-RNA was significantly higher than that of HCV-RNA(χ^(2)=0.893,P<0.01).The results of Pearsen linear correlation analysis showed that HCV-RNA load was significantly positive in HCV-RNA load in HCV patients(r=0.9933,r=0.9732,P<0.001),there was no significant correlation with S/CO value(r=0.3548,P>0.05).Conclusion HCV-RNA qualitative detection is anti-HCV detection is more conducive to the maintenance hemodialysis treatment of patients with HCV infection in early diagnosis of a combination of these can obviously increase the detection rate of HCV infection,at the same time can through the detection of ALT,AST level auxiliary diagnosis of HCV infection.
