摘要
目的:研究老年高血压患者血清25-羟维生素D(25-OHD)水平与血压昼夜节律及夜间脉压指数(nPPI)的相关性。方法:依据血清25-OHD水平,我院的382例老年高血压患者被分为正常组(>20ng/ml,79例)、轻度缺乏组(轻度组,10~12 ng/ml,129例)及重度缺乏组(重度组,<10ng/ml,174例)。比较各组一般临床资料、24hSBP、24hDBP、dSBP、dDBP、nSBP、nDBP、nPPI、nSBP下降率及nDBP下降率。分析血清25-OHD水平与血压昼夜节律及nPPI的相关性。结果:与正常组比较,轻、重度组年龄、BMI、TC、LDL-C水平、24hSBP、dSBP、nSBP、nPPI及重度组24hDBP、dDBP、nDB P均显著升高,轻、重度组nSBP下降率及nDBP下降率均显著降低;且与轻度组比较,重度组年龄、BMI、TC、LDL-C水平、24hSBP、24hDBP、dSBP、dDBP、nSBP、nDBP、nPPI均显著升高,nSBP下降率及nDBP下降率均显著降低(P<0.05或<0.01)。Pearson相关分析显示,血清25-OHD水平与年龄、BMI、TC、LDL-C、24hSBP、24hDBP、dSBP、dDBP、nSBP、nDBP、nPPI呈显著负相关(r=-0.802~-0.247,P均=0.001),与nSBP下降率及nDBP下降率呈显著正相关(r=0.794、0.838,P均=0.001)。多元线性回归分析显示,LDL-C、24hSBP、dSBP、nSBP及nPPI是25-OHD降低的独立危险因素(B=-22.430~-0.074,P<0.05或<0.01),nSBP下降率、nDBP下降率是25-OHD降低的独立保护因素(B=0.295,1.083,P=0.015,0.001)。结论:血清25-OHD水平可显著影响老年高血压患者血压昼夜节律和夜间PPI。
Objective:To study correlation among circadian rhythm of blood pressure,nighttime pulse pressure index(nPPI)and serum level of 25-hydroxyl vitamin D(25-OHD)in aged hypertensive patients.Methods:According to serum 25-OHD level,the 382 aged hypertensive patients were divided into normal group(>20ng/ml,n=79),mild deficiency group(mild group,10~12 ng/ml,n=129)and severe deficiency group(severe group,<10ng/ml,n=174).General clinical data,24hSBP,24hDBP,dSBP,dDBP,nSBP,nDBP,nPPI,decline rates of nSBP and nDBP were compared among all groups.Correlation among circadian rhythm of blood pressure,nPPI and serum 25-OHD level were analyzed.Results:Compared with normal group,there were significant rise in age,BMI,levels of TC and LDL-C,24hSBP,dSBP,nSBP and nPPI in mild group and severe group,significant rise in 24hDBP,dDBP and nDBP in severe group,and significant reductions in decline rates of nSBP and nDBP in mild group and severe group;compared with mild group,there were significant rise in age,BMI,levels of TC and LDL-C,24hSBP,24hDBP,dSBP,dDBP,nSBP,nDBP and nPPI,and significant reductions in decline rates of nSBP and nDBP in severe group(P<0.05 or<0.01).Pearson correlation analysis indicated that serum 25-OHD level was significant inversely correlated with age,BMI,levels of TC and LDL-C,24hSBP,24hDBP,dSBP,dDBP,nSBP,nDBP and nPPI(r=-0.802~-0.247,P=0.001 all),and significant positively correlated with decline rates of nSBP and nDBP(r=0.794,0.838,P=0.001 both).Multifactor linear regression analysis indicated that LDL-C,24hSBP,dSBP,nSBP and nPPI were independent risk factors for decreased 25-OHD level(B=-22.430~-0.074,P<0.05 or<0.01),and decline rates of nSBP and nDBP were its independent protective factors(B=0.295,1.083,P=0.015,0.001).Conclusion:Serum 25-OHD level can significantly affect circadian rhythm of blood pressure and nighttime PPI in aged hypertensive patients.
