摘要
目的探讨高血压患者动态血压参数对颈动脉内-中膜厚度检测意义。方法选取2018年3月—2020年3月在医院初诊的高血压病患者112例,经超声心动图以及颈动脉超声检查后将其分为左室肥厚组39例,非左室肥厚组73例,颈动脉内-中膜增厚组47例,非内-中膜增厚组65例。对所有患者进行颈动脉超声检查、超声心动图检查、动态血压检测、血生化检查及体格检查。结果左室肥厚组24 h平均收缩压、24 h平均舒张压、24 h脉压、白昼脉压、夜间脉压以及动态血压非勺型昼夜节律发生率均高于非左室肥厚组(P<0.05);颈动脉内-中膜增厚组24 h平均收缩压、24 h平均舒张压、24 h脉压、白昼脉压、夜间脉压以及动态血压非勺型昼夜节律发生率均高于非颈动脉内-中膜增厚组(P<0.05)。结论高血压患者的动态血压参数同左室肥厚以及颈动脉内-中膜厚度之具有一定的关联性,当动态血压参数处于不稳定状态时,会提升左室肥厚以及颈动脉内-中膜增厚的概率。
Objective To explore the significance of ambulatory blood pressure parameters in the detection of carotid intima-media thickness in patients with hypertension.Methods 112 patients with hypertension who were first diagnosed in our hospital from March 2018 to March 2020 were selected.After echocardiography and carotid ultrasound examination,they were divided into left ventricular hypertrophy group(39 cases),non-left ventricular hypertrophy group(73 cases),carotid intima-media thickening group(47 cases),and non-carotid intima-media thickening group(65 cases).All patients were examined by carotid ultrasound,echocardiography,ambulatory blood pressure,blood biochemistry and physical examination.Results The 24 h mean systolic blood pressure,24 h mean diastolic blood pressure,24 h pulse pressure,day pulse pressure,night pulse pressure and ambutral blood pressure were all higher in the left ventricular hypertrophy group than in the non-left ventricular hypertrophy group(P<0.05).The 24 h mean systolic blood pressure,24 h mean diastolic blood pressure,24 h pulse pressure,day pulse pressure,night pulse pressure and ambutral blood pressure non-scoops diurnal rhythm in carotid artery-media thickening group were all higher than those of the non-carotid arterymedia thickening group(P<0.05).Conclusion The ambulatory blood pressure parameters of hypertensive patients have a certain correlation with left ventricular hypertrophy and carotid intima-media thickness.When ambulatory blood pressure parameters are in an unstable state,it will increase probability in left ventricular hypertrophy and carotid intima-media thickening.
