大学新生体质量指数与不同亚型高血压的关系

摘要 目的分析大学新生不同高血压亚型现状,探讨体质量指数(body mass index,BMI)与不同高血压亚型的关系,为高血压的早期防控提供科学依据。方法于2017年9月对某大学2017级4653名新生的身高、体质量、静息心率、血压等体检结果进行分析,探讨大学新生体质量指数对不同高血压亚型的影响。结果大学新生收缩压(SBP)、舒张压(DBP)均值分别为(118.08±12.74)mmHg和(69.97±9.50)mmHg(1 mmHg=0.133 kPa),男生SBP、DBP均值高于女生(P<0.01)。大学新生单纯收缩期高血压(ISH)、单纯舒张期高血压(IDH)及收缩期舒张期高血压(SDH)检出率分别为2.15%、1.31%、1.65%,其中男生ISH、SDH检出率明显高于女生(P<0.05),而女生IDH检出率低于男生(P>0.05)。随着BMI水平的增加,男生和女生平均SBP、DBP以及ISH、IDH、SDH检出率逐渐升高(P<0.05);校正年龄、地区、高血压家族史和静息心率等混杂因素后,与正常BMI相比,肥胖组患ISH的风险(OR=6.241,95%CI4.243~8.047),患IDH的风险(OR=7.874,95%CI5.124~10.415)以及患SDH的风险(OR=4.573,95%CI3.653~5.741)均更高。相关性分析显示,男生女生收缩压、舒张压均与身高、体质量及BMI呈正相关(P<0.05)。结论大学新生的血压水平与BMI呈正相关。大学新生不同亚型高血压的检出率和发病风险随着BMI增加而增加,超重和肥胖的大学生发生IDH和SDH的风险更大。 Objective This paper analyzed the current situation of different subtypes of hypertension in freshmen,discussed the relationship between body mass index and different subtypes of hypertension,and provided scientific basis for the early prevention and control of hypertension.Methods The height,body mass,resting heart rate and blood pressure of 4653 freshmen in 2017 were measured to analyze the influence of body mass index on different subtypes of hypertension.Results The mean values of systolic blood pressure(SBP)and diastolic blood pressure(DBP)of freshmen were 118.08±12.74 and 69.97±9.50.The mean values of SBP and DBP of freshmen were higher than those of freshmen(P<0.01).The detection rates of systolic hypertension(ISH),diastolic hypertension(IDH)and systolic diastolic hypertension(SDH)in freshmen were 2.15%,1.31%and 1.65%,respectively.Among them,the detection rates of ISH and SDH in male freshmen were significantly higher than that in female freshmen(P<0.05),while the detection rate of IDH in female freshmen was lower than that in male freshmen(P>0.05).With the increase of BMI level,the detection rate and risk of SBP,DBP,ISH,IDH and SDH of male and female students gradually increased(All P<0.05).After adjusting for age,region,family history of hypertension and res-ting heart rate,the risk of ISH(OR=6.241,95%CI:4.243-8.047),IDH(OR=7.874,95%CI:5.124-10.415)and SDH(OR=4.573,95%CI:3.653-5.741)were all higher in the obese group than those with normal BMI.Correlation analysis showed that SBP and DBP were positively correlated with height,body mass and BMI of both boys and girls(P<0.05).Conclusion Both male and female freshmen’s blood pressure is positively correlated with BMI.BMI level increases the detection rate of different subtypes of hypertension and the risk of morbidity in college students,and overweight and obese college students have a greater risk of IDH and SDH.
