摘要
目的探讨单孔腹腔镜手术治疗小儿化脓性阑尾炎的效果及对免疫指标和炎症反应的影响。方法抽取2018年6月至2020年1月郑州颐和医院收治的化脓性阑尾炎患儿85例,依据治疗方案分为两组。对照组42例给予三孔腹腔镜手术,观察组43例给予单孔腹腔镜手术。比较两组手术指标、免疫指标、炎性指标。结果观察组术中出血量、疼痛持续时间、排气时间、住院时间少于对照组(t=5.584、5.610、5.540、4.070,P<0.05)。术后,两组免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)水平较术前降低(P<0.05),C-反应蛋白(CRP)、白细胞介素-6(IL-6)水平较术前升高(P<0.05);但观察组IgA、IgG、IgM水平高于对照组(t=6.252、4.488、7.136,P<0.05),CRP、IL-6水平低于对照组(t=4.046、5.416,P<0.05)。结论单孔腹腔镜手术治疗化脓性阑尾炎可有效保护患儿的免疫功能,抑制炎症因子释放,促进术后恢复。
Objective To investigate the effect of single port laparoscopic surgery on children with suppurative appendicitis and its influence on immune indexes and inflammatory indexes.Methods A total of 85 children with suppurative appendicitis admitted to Zhengzhou Yihe Hospital from June 2018 to January 2020 were selected and divided into two groups according to the treatment plan.Forty-two patients in the control group were given three port laparoscopic surgery,and forty-three patients in the observation group was given single port laparoscopic surgery.The surgical index,immune index and inflammatory index were compared between the two groups.Results The intraoperative blood loss,pain duration,exhaust time,and hospital stay in the observation group were less than those in the control group(t=5.584,5.610,5.540,4.070,P<0.05).After operation,the levels of immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)in the two groups were lower than those before the operation(P<0.05),and the levels of C-reactive protein(CRP)and interleukin-6(IL-6)were higher than those before the operation(P<0.05);but the levels of IgA,IgG,and IgM in the observation group were higher than those in the control group(t=6.252,4.488,7.136,P<0.05),and the levels of CRP and IL-6 were lower than those in the control group(t=4.046,5.416,P<0.05).Conclusions Laparoscopic single port surgery for patients with suppurative appendicitis can effectively protect the immune function of children,inhibit the release of inflammatory factors,promote postoperative recovery.
