摘要
目的观察不稳定型心绞痛(UAP)患者择期经皮冠状动脉介入术(PCI)后的冠状动脉微循环损伤发生状况。方法抽取2020年1月至2020年12月于河南大学淮河医院接受择期PCI治疗的114例UAP患者作为研究对象,依据患者PCI后有无发生冠状动脉循环损伤状况分为发生组与未发生组。由研究人员设计基线资料调查问卷,询问并记录两组一般资料及实验室指标,通过单因素与多因素分析,找出UAP患者择期PCI后发生冠状动脉微循环损伤的影响因素。结果 114例接受PCI治疗的UAP患者中,有45例患者PCI后发生冠状动脉微循环损伤,发生率为39.47%;发生组合并糖尿病、合并高血压占比高于未发生组,且C-反应蛋白(CRP)和同型半胱氨酸(Hcy)水平高于未发生组(P<0.05);经回归分析结果显示,合并高血压、合并糖尿病、CRP和Hcy水平高均是UAP患者择期PCI后发生冠状动脉微循环损伤的影响因素(OR>1,P<0.05)。结论 UAP患者择期PCI术后发生冠状动脉微循环损伤可能受合并高血压、合并糖尿病、高CRP和Hcy水平等多种因素的影响。
Objective To observe the occurrence of coronary microcirculation injury in patients with unstable angina pectoris(UAP)after elective percutaneous coronary intervention(PCI).Methods A total of 114 patients with UAP who received elective PCI in Huaihe Hospital of Henan University from January 2020 to December 2020 were prospectively selected as the research subjects,and the patients were divided into the occurrence group and the non occurrence group according to whether there was coronary circulation injury after PCI.The baseline data questionnaire was designed by the researchers,and the general data and laboratory indexes of the two groups were asked and recorded,through univariate and multivariate analysis,the influencing factors of coronary microcirculation injury in UAP patients after elective PCI were found out.Results Among the 114 UAP patients treated with PCI,45 patients had coronary microcirculation injury after PCI,the incidence rate was 39.47%;the proportion of combined with diabetes and hypertension in the occurrence group were higher than those in the non occurrence group,and the levels of C-reactive protein(CRP)and homocysteine(Hcy)were higher than those in the non occurrence group(P<0.05);the results of regression analysis showed that combined with hypertension,combined with diabetes mellitus,high levels of CRP and Hcy were the influencing factors of coronary microcirculation injury in UAP patients after elective PCI(OR>1,P<0.05).Conclusions The occurrence of coronary microcirculation injury in UAP patients after elective PCI may be affected by many factors such as combined with hypertension,combined with diabetes mellitus,high levels of CRP and Hcy.
