摘要
目的:分析肌电图辅助诊断腰椎间盘突出症的临床价值。方法:2019年1月-2020年3月收治腰椎间盘突出症患者60例,作为研究组;同期选取正常体检者60例,作为对照组。两组均进行肌电图辅助检查,分析腰椎间盘突出症患者肌电图检查结果,比较两组肌电图检查相关指标水平。结果:研究组患者肌电图检查显示异常55例,正常5例,临床符合率为91.67%;其中肌肉、神经传导均异常37例(37.2%);检测肌肉312块,异常50例(83.33%);检测出感觉神经传导233条、运动神经传导300条,异常38例(63.33%),其中感觉神经传导异常16例(50.00%),运动神经传导异常22例(68.75%)。研究组患者腓总神经、股神经及胫神经的复合肌肉动作电位潜伏期均长于对照组,而运动传导速度(MCV)均慢于对照组,差异有统计学意义(P<0.05);研究组患者胫前肌、股四头肌及趾短伸肌的时限均长于对照组,且电压均高于对照组,差异有统计学意义(P<0.05)。结论:肌电图作为辅助诊断腰椎间盘突出症的重要方法,可以有效判断患者神经损害程度,为临床诊治提供可靠的参考依据。
Objective:To analyze the clinical value of electromyography in assisting the diagnosis of lumbar disc herniation.Methods:From January 2019 to March 2020,60 patients with lumbar disc herniation were selected as the study group.60 patients with normal physical examination were selected as the control group during the same period.Both groups underwent electromyographic auxiliary examination,we analyzed the results of electromyographic examination of patients with lumbar disc herniation,and compared the levels of related indicators of the electromyographic examination of the two groups.Results:The electromyography of the study group showed abnormalities in 55 cases and 5 normal cases.The clinical coincidence rate was 91.67%.37 cases(37.2%)were abnormal in both muscle and nerve conduction.312 muscles were detected,50 cases were abnormal(83.33%).233 sensory nerve conduction and 300 motor nerve conduction were detected,38 cases(63.33%)were abnormal,among them,sensory nerve conduction was abnormal in 16 cases(50.00%),motor nerve conduction was abnormal in 22 cases(68.75%).The latent periods of compound muscle action potentials of the common peroneal nerve,femoral nerve and tibial nerve in the study group were longer than those in the control group,and the motor conduction velocity(MCV)was slower than the control group,the differences were statistically significant(P<0.05).The duration of the tibialis anterior muscle,quadriceps muscle and extensor digitorum brevis muscles of the study group was longer than that of the control group,and the voltage was higher than that of the control group,the differences were statistically significant(P<0.05).Conclusion:Electromyography can be used as an important method to assist in the diagnosis of lumbar disc herniation.It can effectively determine the degree of nerve damage in patients and provide a reliable reference for clinical diagnosis and treatment.
