摘要
目的探讨影响脑卒中患者创伤后成长的因素并分析其之间相关性。方法采用便利抽样法,分别用心理弹性量表(connor davidson-resilience scale,CD-RISC),希望水平量表(herth hope index,HHI)、创伤后成长量表(post traumatic growth,PTGI)对山东省滨州市3所三级甲等医院的脑卒中患者进行调查,数据统计分析采用SPSS 25.0。结果希望水平得分为(39.30±3.61),心理弹性得分为(55.40±6.26),创伤后得分(68.92±5.54)。希望水平得分与心理弹性得分(r=0.566,P<0.001),创伤后成长得分均呈现相关(r=0.401,P<0.001),心理弹性得分与创伤后成长呈正相关(r=0.490,P<0.001)。不同婚姻状况、家庭收入、卒中时间的脑卒中患者创伤后成长评分比较差异有统计学意义(P<0.05)。多元线性回归分析结果显示:婚姻状况(β′=0.118)、家庭人均收入(β′=0.202)、卒中时间(β′=0.134)、心理弹性评分(β′=0.387)、希望水平(β′=0.181)为脑卒中患者创伤后成长的影响因素(P<0.05)。结论脑卒中患者希望水平处于中等水平,心理弹性水平高的病人其希望水平可能相应较高,从而提高患者的创伤后成长水平。护理人员要挖掘其心理弹性,提高患者的希望水平,促进其身心健康的恢复。
Objective To explore the factors that affect the post-traumatic growth of stroke patients and analyze their correlation.Methods With convenience sampling,the connordavidson-resilience scale(CD-RISC),herthhope index(HHI)and post traumatic growth(PTGI)were respectively used to investigate the stroke patients in three Class A tertiary comprehensive hospitals in Binzhou,Shandong Province.The data were statistically analyzed by SPSS 25.0.Results The scores of hope level,psychological resilience and post-trauma were(39.30±3.61),(55.40±6.26)and(68.92±5.54).The score of hope level was correlated with that of psychological resilience(r=0.566,P<0.001)and post-traumatic growth(r=0.401,P<0.001).The score of psychological resilience was positively correlated with that of post-traumatic growth(r=0.490,P<0.001).There were statistically significant differences in the score of post-traumatic growth of stroke patients with different marital status,family income and stroke time(P<0.05).The results of multiple linear regression analysis showed that marital status(β′=0.118),family income per capita(β′=0.202),stroke time(β′=0.134),score of mental resilience(β′=0.387)and hope level(β′=0.181)were the influencing factors of post-traumatic growth of stroke patients(P<0.05).Conclusion The hope level of stroke patients is at a moderate level,and the hope level of patients with high mental flexibility may be correspondingly higher,thereby increasing the level of post-traumatic growth of patients.Nursing staff should excavate their psychological resilience,improve the patient s hope level,and promote the recovery of their physical and mental health.
